Service Coordinator/QIDP
Plainview Helen Kaplan Program House C

Mon - Fri  Flex  35 hrs per week

Qualifications:

  • Bachelor's Degree in Human Services or related field plus one year experience supporting individuals with intellectual and other developmental disabilities
  • Prior QIDP experience preferred
  • Excellent oral and written communication skills
  • Must be a team player.  

Primary duties and responsibilities include but not limited to: 

  • Practices and promotes Citizens mission statement in every interaction
  • Oversees the development of a CFA functional assessment for each person by defining capabilities, deficits and needs through the use of assessments to insure that each person is receiving the clinical habilitative service they require
  • Supervises and monitors the implementation of each person’s program plan through daily interaction with staff and all people. Reviews progress, utilizes communication log books, and observes a person in their daily routine in order to continue, modify or revise the program plan to ensure that the person’s needs are adequately met and that services are received
  • Serves as an advocate on behalf of the person by observing them in their daily routine in order to identify and assess need areas, as well as to protect their rights
  • Monitors goal, medical and record books by reviewing data, programs, evaluations and consultations to ensure accuracy, consistent and holistic service plans which meet regulatory compliance
  • Serves as the primary family contact and documents contact as received and made. Addresses family concerns and questions and provides on-going information.  Leads family group meetings to keep family members informed of program changes, provide educational information and emotional support through utilizing group work skills, current information and guest speakers
  • Collaborates with other professionals at day programs and in the community via mail, fax, phone and/or meetings to exchange information and ensure continuity of care regarding a person
  • Participates in the development and ongoing monitoring of psychiatric caseload to include monthly participation in meetings, daily review of data and observations. Ensures regulatory compliance to protect people’s rights
  • Reports and documents any events/activities that may hinder the health, safety and well-being of the person that reside there (i.e., allegations of abuse, injuries, broken equipment, etc.)
  • Reports to the Justice Center as a mandated reporter if there is reasonable cause to suspect abuse or neglect. Additionally, must follow all agency incident policy and procedures
  • Adheres to Federal and New York State regulations, agency policy and procedures, the employee handbook and agency and program training requirements
  • Successfully completes and maintains annual training and any other training that is necessary for the position
  • Arrives to work by the start of scheduled shift, adheres to agency policy and procedures in regard to call offs and overtime to ensure quality support is provided to people each day
  • Mentors and acts as a role model for new Direct Support Staff by demonstrating supportive routines and by reviewing training manuals to ensure new employees are sufficiently groomed to complete responsibilities
  • Participates in the development and ongoing monitoring of people reviewed by the Human Rights Committee and monthly Psychiatric Meetings. Assists in the development of quarterly documentation and ensures action is taken on committee recommendations
  • Ensures a person receives and maintains all entitled benefits by overseeing the documentation process to ensure needs are met
  • Provides in-service training for new employees on people’s rights, human growth & development and working with families. Provides other training as indicated to insure staff knowledge to better meet a person’s needs
  • Serves as a member of the admission/discharge committee by attending screenings and participating in all meetings to offer recommendations that will insure a smooth transition for people. Coordinates admission and discharge plan and system
  • Develops and updates psycho-social histories by clarifying family histories and reviewing a person’s personal files to reflect an individual’s current level of functioning
  • Performs other duties as requested by supervisor
  • Ensure that work-related safety procedures are in place and are being adhered to by staff
  • Observance of an unsafe act or a potentially hazardous work environment must be immediately reported to supervisor and/or safety committee

This position requires regular and substantial unsupervised or unrestricted physical contact with people receiving services and, therefore, is subject to successful completion of fingerprint/criminal background check.
