The RN Care Coordination Team Lead (RNCC TL) serves as a clinical and administrative support to the Care Coordination Manager to ensure the delivery of quality care, as well as continuity of care for patients and families. The RNCC TL provides leadership to Care Coordination team members in order to ensure excellence in care transitions. The RNCC TL works with the Care Coordination Manager to provide staff development and assist with performance evaluations, and works collaboratively with clinical leadership to facilitate throughput. The RNCC TL acts as a resource to the team, and provides clinical leadership to achieve organizational and departmental goals.
• Provides operational and clinical guidance to the Care Coordination staff. • Ensures touchpoints with new staff members to evaluate the orientation process and progression through the training plan. • Observes and coaches staff in performance of assigned duties and interpersonal interactions. • Provides ongoing assessments, evaluations of staff performance, and educational support to all staff within the department. • Assisting leadership in developing staffing plans that meet departmental needs. • Coordination of staffing, schedules, and coverage for assigned units to include weekend and holiday coverage. • Utilize the escalation process to accurately and timely notify Care Coordination leadership of any arising issues. • Supports leadership in ensuring compliance with Joint Commission, federal, state and other applicable regulatory requirements. Implements program philosophy, objectives and standards of care on consultation with leadership. • Effectively communicates with all staff in order to maintain a cohesive team dynamic • Development and maintenance of professional relationships with the hospital interdisciplinary care team, physicians, and post-acute care provider networks. • Supports Care Coordination Manager with planning and executing internal projects and implementing programs in a timely manner within the team.
