Job Description
Coordinate the patient/family care and services including those of the IDT. (interdisciplinary team)
Appropriately utilizes the skills and resources of contract personnel such as physical therapy when appropriate.
Develops and updates plan of care with the IDT and others involved in the patients care.
Provides skilled nursing as determined by the plan of care.
Educates patient/family and other primary caregivers, in appropriate care techniques.
Serves as the patient/family advocate by maintaining regular communication with attending physicians, long term care facility staff, HMO case managers, and others as well as .
May participate in providing bereavement support to families and associated bereavement activities.
Ensure and document appropriate use of medication, durable medical equipment and supplies
Participates in team meeting.
Provide appropriate, coordinated care plans.
Monitors the recertification/reauthorization process
Reports necessary information to the Team Leader and other members of the IDT for on-call shift report.
Responds to on-call issues as clinically appropriate
Performs on-site evaluations and supervision of Grace HHAs.
Qualifications
Currently licensed to practice nursing in the state
A minimum of two years nursing experience in an acute-care hospital nursing in either medical, surgical or oncology unit or other commensurate experience.
