Qualifications:
Master’s degree in Social Work required
1-2 years experience in a healthcare setting, preferred
Current LICSW license through the Minnesota Board of Social Work
BLS certification
Job Responsibilities:
Assists patients and their families with concerns that accompany illness or inhibit recovery and rehabilitation.
Interviews, assesses/screens, diagnoses, and treats patients in need of mental healthcare.
Assists with making the necessary arrangements for leaving the hospital or clinic, such as nursing home placement or home health services, as backup to Medical Social Services.
Initiates necessary referrals to appropriate community resources and provides crisis intervention as needed.
Attends organizational committees and work groups as assigned by supervisor.
Establishes and completes at least one quality improvement project each year.
Promotes staff development by creating an environment of learning and rediscovery.
Acts as a community liaison.
Must have reliable transportation and a current driver’s license.
