Responsibilities
Perform personal care needs
Plan and prepare nutritious meals and assist in feeding the patient when necessary
Take and record temperatures, pulse, respiration, and blood pressure when ordered with appropriate completed/demonstrated skills competency when on patients plan of care
Assist in ambulation and exercise, as instructed
Assist patient in self-administration of medication
Meet safety needs of patient and use equipment safely and properly
Report on patient’s condition and significant changes to the assigned nurse
Keep the patient’s living area clean and orderly
Provide respite for patient’s family/caregiver when on-site, as appropriate
Ensure compliance with all state and federal legal and regulatory requirements
Additional responsibilities as assigned
Qualifications
Meets the training requirements of the State
Completed a minimum of seventy five hours of classroom and supervised practical training Sixteen hours of classroom must be completed prior to a mandatory sixteen hours of practical training
Minimum of six months experience including providing personal care
At least eighteen years of age
Ability to read and follow written instructions and document care given
Understands hospice philosophy, comfortable providing specialized care to the terminally ill
Satisfactory references from previous employers
Ability to work with little direct supervision
Secure with issues of death/dying
Provides a calm manner when in a patient’s home
Possess and maintains current CPR certification
Demonstrates tact, patience, and good personal hygiene
Successfully completes a probationary period
This probationary period is to ensure the aide is competent in the above qualifications and is documented by the supervisor and the employee
Strong oral and written communication skills
Good organizational skills
