What You’ll Do

This is a new position for a Home Hemodialysis Registered Nurse (RN) to support our growth into a new, innovative home dialysis program in the Chicago metropolitan area.

The Home Hemodialysis Registered Nurse (RN) will be responsible for delivering high-quality patient-centered care by educating, training, and supporting patients who perform dialysis in their homes. This role requires a strong clinical background, excellent communication skills, and the ability to build trusting relationships with patients and their caregivers.

As a critical part of our care team, the Home Hemodialysis RN will empower patients to manage their treatment safely and effectively while ensuring adherence to clinical and regulatory standards.

All duties and responsibilities are expected to be performed in accordance with company policy, procedures, standards of nursing practice, state and federal regulations and as directed by the appropriate chain of command.

The role will serve the general Chicago area and be based near Joliet. Role requires daily local travel in the Chicago metropolitan area (approx. 60-mile radius) to perform patient training and assessments in their home environment.

Minimum Qualifications:

Education and Experience:

  • Graduate of an accredited School of Nursing.
  • Current appropriate state licensure.
  • Minimum of 12 months current experience in hemodialysis
  • Current or successful completion of CPR BLS Certification.
  • Must meet the practice requirements in the state in which he or she is employed.
  • Must meet the state or facility health mandates, if applicable.

Required Skills and Competencies:

  • Strong knowledge of dialysis operating requirements.
  • Strong clinical assessment and critical thinking skills.
  • Excellent communication, teaching, and interpersonal skills.
  • Ability to work independently and manage multiple patients in a remote setting.
  • Proficiency with electronic medical records (EMRs) and Microsoft Office Suite.

Essential Duties and Responsibilities:

  • Train patients and caregivers on home hemodialysis procedures, including equipment setup, treatment initiation, monitoring, and troubleshooting.
  • Provide comprehensive education on infection control, vascular access care, and emergency protocols.
  • Conduct initial patient assessments, including medical history reviews and home environment evaluations, to determine suitability for home dialysis.
  • Develop and implement individualized care plans in collaboration with the interdisciplinary team.
  • Provides routine training, education and guidance to the patient and patient’s caregiver as needed on varying topics.
  • Monitor patient progress through routine follow-ups and assessments, adjusting care plans as necessary.
  • Maintain accurate and timely documentation in compliance with state and federal regulations, including CMS guidelines.
  • Ensure that patient care aligns with organizational policies and quality standards.
  • Train patients in the maintenance and use of dialysis equipment, ensuring proper functioning and compliance with manufacturer guidelines.
  • Educate, train, and help coordinate delivery and inventory management of necessary supplies.
  • Serve as a resource and advocate for patients and caregivers, addressing concerns, troubleshooting issues, and providing emotional support.
  • Facilitate communication between patients, physicians, and other healthcare providers to promote continuity of care.
  • Provide ongoing education and preparation for managing complications such as hypotension, bleeding, or equipment malfunction during dialysis treatments.
  • Participation, as directed, in Quality Assurance & Performance Improvement or CQI.
  • Payroll: Records both arrival and departure time via ADP’s time and attendance system.
  • Promotes and follows the Company’s mission, vision, and core values.
  • Demonstrates a high degree of self-motivation and willingness to improve.
  • Openly receives and promptly acts upon feedback from supervisors, peers, and clients.
  • Reports incidents immediately to supervisor.
  • Regularly checks work email and responds appropriately and timely.
  • Maintains regular and predictable attendance & punctuality.
  • Performs other duties as assigned.
  • Ability to travel to patients' homes across the assigned state.
  • Ability to work remotely and in an office setting when required.
  • Valid driver’s license and reliable transportation.
  • Ability to lift and move dialysis equipment and supplies, as required.
  • Ability to be flexible and adhere to innovative scheduling.
  • Ability to take on-call shifts as needed.

Supervision:

  • Initially reports to VP, Clinical Quality and ultimately reports to the lead for Home Dialysis Services.

Work Environment:

This job exists in a hospital, training center, patient residence, or in the medical unit of a post-acute or correctional facility. This role may include exposure to chemicals and blood-borne pathogens. This role routinely involves the use of both medical and standard office equipment.

While performing the duties of this job, the employee is regularly required to talk or hear. The employee frequently is required to stand; walk; use hands to handle or feel; and reach with hands and arms. This position requires the ability to push, pull or lift supplies and products up to 50 pounds. Specific visual abilities required by this job include close vision and the ability to adjust focus.
