East Bay Community Action Program (EBCAP) is seeking a full time Case Manager for our Behavioral Health Services Division at our 2 Old County Road location in Barrington, RI.

Key Responsibilities:

Perform a variety of case management and care coordination duties on our Assertive Community Treatment (ACT) Team, relating to complete case management in support of clients living with Severe and Persistent Mental Illness (SPMI) who are living independently in the community, both community-based, and hospitalized.

Provide direct assistance, advocacy, coordination of care, side-by-side individualized support, and/or problem-solving skills, to help clients obtain medical and dental health care (individual and family support services).

Qualifications:

• Bachelor's degree required
• Knowledge and proficiency in community-based treatment and rehabilitation.
• Ability to work a rotating 4-hour shift on weekends/holidays.
