Memory Wellness Program Facilitator
Summary:
The Memory Wellness Program Facilitator will be responsible for the development, coordination, and implementation of programs and services for residents with cognitive impairment on the Wesley Willows campus, primarily in the Kirk’s Place Memory Care Unit, ensuring a therapeutic environment that maintains the highest level of well-being of the body, mind, and spirit of each resident, meeting their interests and capabilities.
Essential Duties and Responsibilities include, but are not limited to:

Assumes the responsibility, under the direction of the Director and Asst. Director of Memory Wellness, for the design and development of recreation and leisure programs to meet the assessed needs, interest, and capabilities of the residents.

Assesses and documents resident’s abilities and needs and records behavior and participation in activities so this can be analyzed in relation to needs, interests, and past lifestyle.

Assures that all activities conform to the criteria of federal, state, and local accreditation and licensing agencies or organizations.

Compiles a weekly activity program calendar, has the calendars printed, and distributes them to appropriate personnel and residents.

Coordinates programs with other departments and interfaces with all personnel as appropriate.

Uses community resources in the development of programs.

Relates in a positive manner to residents and maintains positive behavior at work, delivering quality care and compassion to residents.

Demonstrates professionalism and pride in performance.

Exhibits a willingness to adapt to changing environments; flexible in work assignments and demonstrates ability to perform as a part of the corporate team.

Maintains a safe and productive work environment.

Orients and trains new employees in activities, acting as a positive teammate by assisting in onboarding and the support of unit staff.

Utilizing creativity and improvisational skills to implement stimulating experiences and activities.

Protect and promote residents’ rights in all interactions with families and residents being a role model for the unit.

Participate with the interdisciplinary team in the development and maintenance of the social service and activity portions of the plan as a member of care for each resident.

Administer social history assessments to support a person-centered approach to care.

Participate in continuing education opportunities to ensure up-to-date best practices of memory care.

Performs all other special assignments as required by the [Assistant] Director of Memory Care.
Minimum Education/Skills/Experience/Credentials:

Minimum of one (1) year of experience in Senior Living/Dementia/Rehab or Long-Term Care.

Highschool Diploma or GED required, 2 or 4-year completed degree in Healthcare, Human Services, Occupational/Physical Therapy, or related field preferred.

Active IL CNA certification required.

Ability to manage multiple priorities.

Excellent interpersonal relations with proven communications skills, both verbal and written.

Proficiency in computer skills, including Microsoft Office (Windows, Outlook, Excel), with the ability to learn new applications.

Competent in organizational and time management skills.

Demonstrates good judgment, problem-solving, and decision-making skills.
Other regulated requirements:

Must be willing to undergo pre-employment background check, including (MVR) moving vehicle records.

Must have or be willing to receive the Flu-Shot Vaccine, and TB testing.

Must be willing to undergo pre-employment drug screening, including cannabis.

Must be willing to undergo biometric screening.
