RESPONSIBILITIES INCLUDE but not limited to:

  1. Develops care plans appropriate to the patient’s health care needs in conjunction with the UR Medicine Home Care (URMHC) Quality team, patient, family, physician, and third party payers including obtaining medical orders.
  2. Completes the OASIS Assessment to accurately reflect the clinical, functional and psycho-social needs of the patient and other skilled visits as assigned.
  3. Communicates to the health care team members the outcome of the assessment/reassessment (if applicable) visit to ensure appropriate follow up occurs based on the needs of the patient/family’s plan of care.
  4. Ensures problem resolution at the point of contact.

QUALIFICATIONS:

  1. New York State Licensed Registered Nurse.
  2. Working knowledge of health care reimbursement systems, government regulations and health industry accreditation standards relative to health care eligibility criteria, payer utilization, prior authorizations, and assessment/teaching standards.
  3. Minimum of two-year’ experience in community health experience preferred.
  4. One year of OASIS experience required
  5. Ability to effectively communicate with all internal and external health care team members and insurers.
  6. Excellent organizational/interpersonal skills (including time management, patient visit prioritization).
  7. Ability to effectively assess the physical and psycho-social needs of the patient/family and develop an appropriate plan of care.
  8. Willing to utilize the technology as appropriate to meet the requirements of the job functions.

Salary Range: $73,652 to $86,548
