Care Coordinator Supervisor (Fulton, Henry, Williams, Putnam, Defiance, Paulding, Van Wert and Mercer counties)

Unison Health is seeking a full-time Care Coordinator Supervisor to lead a team of individuals in the Ohio Rise Program. This position is responsible for promoting the recovery of children/families participating in the Ohio Rise program through the supervision of direct service care coordinators in the following counties: Fulton, Henry, Williams, Putnam, Defiance, Paulding, Van Wert and Mercer.

 Education/Experience/Other Requirements:

  • Bachelor’s degree in social work, psychology or related field with a master’s degree preferred.
  • LSW, LPC, LISW or LPCC preferred.
  • Minimum three years’ experience in children's behavioral health, child welfare, developmental disabilities, juvenile justice, or a related public sector human services or behavioral health care field providing community-based services to children and youth, their family, or caregivers.
  • Must be proficient in the use of computers and software.
  • Possess a valid driver’s license, reliable transportation, be able to be insured through the agency commercial policy and have personal auto insurance.
  • Prior supervisory experience preferred.

Duties:

  • Implement the required model of care, provide individualized care based on client need, and link clients to needed services and supports. 
  • Ensure assigned staff participate in ongoing training and coaching to ensure consistency in delivering care coordination.
  • Monitors completion of paperwork by assigned staff, including initial assessments, CANS assessments, care plans, and discharges.
