High Tech Nurse, LPN – Home Health

Part Time (24 hours)

Available shifts include Day, Evening, or Overnight

Are you a High Tech Nurse or looking to increase your skill set? Come make a difference in a family’s life. We have many lovely clients who live with their families at home, children and adults who are medically and physically fragile.  Many are on ventilators or BiPap, need respiratory therapy and airway clearance, who have feeding pumps, G-tube/J-tube medications, need Physical Therapy, physical assessments, may have Special Education linked to the school system and more. High Tech Home care offers you a place to learn and/or use a wide variety of skills that you don’t get in most jobs.   We value your experience but are willing to train the right candidate!

We are seeking LPNs to join our interdisciplinary team providing skilled nursing services to clients in their home. We offer an intensive mentoring program upon hire and ongoing education. We have a variety of schedules available to help you have a work/life balance including: part-time, full-time, and various per diem positions.

What we do: Whenever possible, children and their families are given the choice to live in their own homes and communities, not hospitals. Some children live with medical conditions that require in-home nursing care to achieve this goal. Pediatric High-Tech Nursing enables medically fragile children with complex needs to be cared for safely at home and in their community. Adult High Technology services provides skilled nursing care to people age 21 and over living in home-based settings who are eligible for Medicaid and depend on medical technology to survive. This role provides the opportunity to develop long-term meaningful relationships with patients and their families, to advocate for the patient, and to feel like you are making a difference in the quality of their lives.

Our care is family centered, age specific and culturally appropriate. This role enables patients to live in the setting of their choice, and is integral in keeping individuals in their homes. Our shifts cover days, evenings, nights, weekends and holidays. The HTN develops meaningful relationships with their patients and provides much needed hands on care.

The High Tech Licensed Practical Nurse, LPN will provide skilled high tech nursing care to technology-dependent adults and/or children with complex medical conditions in their home and in conjunction with the established plan of care including ongoing assessments of the patient's physical and psychosocial health status.

Qualifications

  • Accredited School of Nursing LPN Program
  • Two or more years of directly related home/public health, hospice and/or maternal/child health nursing experience
  • Current LPN licensure or compact licensure recognized by the State of Vermont required
  • Previous home care desired but not required. We are willing to train and develop a candidate who demonstrates a willingness and desire to learn skilled nursing.
