Position Overview:

The Case Planner enhances the quality of life for youth residing in the Supervised Independent Living Program (SILP) located in the Bronx. The program provides housing and support services to youth aged 18-21. The Case Planner will engage youth effectively using Motivational Interviewing, Conflict Resolution, Mediation and other treatment modalities when delivering services and building relationships with youth as they prepare to transition to independent living. The Case Planner will provide services and/or arrange for the provision of social and related services based on service plans, needs and goals as identified through intake and assessments. Case Planner working closely with Youth Advocates will prepare youth for independent living, help to identify supportive housing, affordable housing or other stable housing options including family reunification. Case Planner will maintain compliance with Children’s Village, ACS and OCFS standards, policies and procedures.

Position Qualifications:

Bachelor’s degree in social work or criminal justice from an accredited college. Must have at least three years’ experience working with foster care youth, homeless youth/young adults, juvenile justice involved youth or other disadvantaged populations. Familiarity with ACS/OCFS programming. Must understand the critical nature of timely and accurate documentation of service delivery and case work practice. Must have knowledge of community resources and skills to link young adults with appropriate agencies and services. Must be a team player and have the ability to work independently. Ability to interact, and work with culturally diverse individuals.

Salary: $32.96 hourly (Flat Rate)
