Care Givers will need to demonstrate reliability, initiative and professionalism as well as a caring, supportive
and nurturing behavior. They must provide high quality resident care in accordance with established policies
and have the skills necessary to manage the resident services and hands-on activities required by the residents
and the nature of the community.
KEY POSITION SPECIFIC ROLES & RESPONSIBILTIES
Meet with team lead at the end of each shift to inform them about resident care, resident changes, and
all important resident information
Inform team lead of any resident emergency situations in a timely fashion and complete proper
documentation before shift end on appropriate forms; notify on-call administration, in the absence of a
team lead
Review and be familiar with resident service plan and history as appropriate to ensure quality care for
each resident
Encourage socialization among residents; review and be familiar with resident chart as a tool to learn
resident’s social history and interests and review with all caregivers,
Review daily assignment sheets for changes at the beginning of each shift
Complete all duties on daily assignment sheet and sign-off when completed
Complete time sheets daily and accurately
Assist with the admission, discharges and transfers of residents as assigned
Assist with the hands-on care of residents such as bathing, hygiene, dressing, undressing, laundry and
other activities of daily living
Report any maintenance/building or safety issues/problems to front desk personnel or complete
maintenance work order in the absence of front desk personnel
Assist in the dining room at meal times, serving residents, busing tables, clean-up, room set-up, and
room trays as assigned
Follow all Quality Standards and Core Values
Answer call lights promptly and complete proper documentation
Provide escorts for residents to and from meals and activities as needed
Report any physical, mental or emotional changes of residents to Nurse., bruises, skin tears,
depression, anxiety, lack of appetite, etc.
Attend and participate in regularly scheduled staff meetings and trainings
Promote harmony and cooperation between shifts and buildings
Responsible for the transfer and security of all community equipment, including pager, radio, keys, etc,
Follow chain of command when reporting concerns or incidents
Must speak English when having direct contact with residents and in all common areas
Maintain confidentiality according to HIPAA standards
Other duties as assigned by the Executive Director

MINIMUM JOB REQUIREMENTS
High school diploma or GED preferred.
Experience in assisted living setting and multi-line phone system preferred but not required.
