Job description

The ARC of the South Shore is currently looking for RN’s to conduct in-home evaluations for the MassHealth PCA program. Pay is per visit. Several geographic areas available.

Sign on bonus $3,500 for Full Time (30-40 hours), $1,750 for Part Time (20-29 hours) and $1,750 for Fee Based positions. The bonus will be paid out in two increments!

EDUCATION/EXPERIENCE/TRAINING:

Graduate of an accredited Registered Nursing Program; experience working with individuals with disabilities

JOB SUMMARY: Under the direction of the PCM Director, the RN Evaluator has an expertise in the Masshealth Regulations regarding time for tasks for Personal Care for the Masshealth Member/Consumer. He/She utilizes these skills in their work with the members (Consumers) to identify and assess their functional abilities and reports this information on the Masshealth template provided by the PCM Director at the time of the evaluation. When performing an initial evaluation, The RN Evaluator will schedule with the member/consumer and the Occupational Therapist and, according to Masshealth Regulations, will supervise such evaluation.

ESSENTIAL JOB DUTIES AND RESPONSIBILITIES FOR THE PCM FEE BASIS RN Evaluator POSITION include the followingOther duties may be assigned. Employees must be able to perform the essential functions of the job with or without reasonable accommodation.

1. Conduct re-evaluations and/or supervise and conduct all initial evaluations with the Occupational Therapist as assigned on the Masshealth template provided by the PCM Director within the Masshealth guidelines and appropriate timelines.

2. Schedule evaluations at the Consumer’s home as assigned, as promptly as possible and report all issues with member/consumer contact to the appropriate skills trainer after the third consecutive try, if unsuccessful.

3. Travel to and from Consumer homes and conduct functional assessments during the in home visit with the Consumer.

4. Complete and submit accurate billing and payroll information to the PCM Director on a weekly basis.

5. Maintain required productivity.

6. Participate in job related training opportunities as they become available.

7. Establish and maintain professional relationships with Consumers/Surrogates, Co-workers, Supervisors and Collaterals

8. All other job related duties as assigned by the PCM Director.

Acceptable CORI, Driver History and Fingerprint results required.
