Join our client's team as an Enrollment Registered Nurse for the PACE Program in Los Angeles. As an Enrollment Registered Nurse, you will help to bring the PACE (Program of All-Inclusive Care for the Elderly) program to serve frail elders and disabled older adults in underserved communities across Los Angeles.
The Enrollment Registered Nurse is a key player of the Outreach Team as we find and assess prospective participants for our PACE program. This person acts as a liaison between the IDT (Interdisciplinary Team) and the Outreach Team to ensure a smooth enrollment process for the prospective participants.
- Full Time Permanent Position
- Monday-Friday, 40 Hours a Week
- Mostly in-field home appointments and occasional onsite work in Los Angeles clinic up to 5 days per month.
- 95% Travel to Patient Homes
- 3-5 Visits/Day Dependent on Enrollees

Responsibilities
- Collaborate with Outreach Specialist and IDT to ensure successful enrollment
- Conduct home visits to evaluate the prospective participant for potential eligibility for nursing home level of care
- Conduct home safety evaluations of the prospective participants’ home
- Participate in Enrollment Eligibility meetings to review Level of Care recommendations
- Complete Nursing Facility Level of Care (NFLOC) assessments and submit to DHCS for review; update/revise the assessment as requested by DHCS
- Maintain accurate documentation and record all evaluations in CRM software
- Participate in all PACE Audits by DHCS and CMS
- Support the Center Director to develop participant retention strategies
- Maintain knowledge and expertise in eligibility, enrollment, and program specifications for the PACE Program
- Support business development and census growth by participating and assisting with outreach programs as needed
Requirements
- Unrestricted California Registered Nurse license
- Experience working in Geriatric or Senior Care
- Passion and mission orientation for serving high-risk seniors and frail older adults
- In-Home experience preferred
- Enrollment experience a plus
- CPR and First Aid certification (within 60 days of employment)
- Driver’s License
- Reliable transportation
