Hourly Pay Range: $21.00 - $24.50

The Residential RN provides medical case management for individuals with developmental disabilities who are living in community based homes. Supervises the adequate completion of delegated nursing tasks to Direct Support Staff and LPNs. Provides/participates in agency training.

ESSENTIAL FUNCTIONS/RESPONSIBILITIES:

  • Participates as a member of the Interdisciplinary Team to provide nursing recommendations related to the individuals overall plan of care.
  • Monitors and coordinates the delivery of preventive and routine health care for individuals with developmental disabilities.
  • Adheres to and ensures facility compliance with all Physicians, NP, PA, etc. orders.
  • Ensures adequate scheduling and completion of health related exams, appointments, laboratory/ diagnostic testing as ordered or required.
  • Accompanies individuals on appointments, if deemed necessary.
  • Assists the site management in the coordination of medical program needs with community services, medical providers and other related agencies.
  • Coordinates and provides health services training for staff.
  • Completes nursing responsibilities as indicated on the Nursing Responsibility Checklist for Residential Programs.
  • Monitors for completion of delegated health related tasks to Direct Support Staff and LPNs.
  • Based upon the independent judgment of the RN, the RN has the authority to suspend and/or revoke the responsibility of a Direct Support Staff or LPN to perform or complete any delegated task.
  • Monitors medication administration in the facility.
  • Flexes schedule to provide medication practicums as required by regulation for Direct Support Staff and LPN as needed or deemed necessary by RN Supervisor.
  • Supervises medication administration responsibilities completed by Direct Support Staff and LPNs, and ensures that staff who are passing medications are current in their certification.
  • Evaluates staff medication errors. Ensures that the plan of correction and retraining follows the Medication Error Point System.
  • Based upon the independent judgment of the RN, the RN has the authority to suspend and/or revoke medication certifications and medication administration of the Direct Support Staff or LPN.
  • Completes a thorough Semi Annual Medication review (Non-ICF) or ensures a Pharmacist completes a Quarterly Medication Review.
  • Completes (Quarterly-ICF) Nursing review on each individual.
  • Updates the Plans of Nursing Services as health status and diagnosis change.
  • Within each month, completes a monthly breast exam on female individuals if ordered.
  • Utilizes technology and software approved by the Agency to provide care planning and documentation related to individual care.
  • Provides phone triage to sites on caseload during working hours.
  • Flexes schedule to provide services to individuals/sites as deemed necessary by the RN Supervisor.
  • Participates in various nursing workgroups as assigned.
  • Maintains compliance with all applicable State/Federal regulations and agency policy and procedure.
  • Communicates issues and carries out all directives from Nursing Administration.
  • Presents to the assigned site when notified that state or federal survey has arrived.
  • Participates in any plan of correction related to medical citations resulting from any internal or external survey.
  • Maintains communication with LPN (as applicable), Senior Residential Supervisor, residential managers, Service Coordination, Nursing Supervisor, and other staff as necessary.
  • Provides nursing coverage to all People Inc programs as directed by Nursing Administration.
  • Maintains CPR certification and performs CPR when necessary.
  • Provides agency training related to nursing services as required.
  • Complies with all agency policies and procedures.
  • Other duties as assigned by Nursing Administration.
MINIMUM QUALIFICATIONS:
  • Current NYS license to practice as a Registered Nurse.
  • NYS Drivers License that meets agency policy.
  • CPR certification.
  • Physical agility and ability to react to emergency situations, including Strategies for Crisis Intervention and Prevention (SCIP-R) certification as per regulatory guidelines.
  • Ability to meet physical requirements of the position when performing the following: Standing, walking, sitting, bending, stooping, squatting, kneeling and climbing.
