Qualifications:
- Master’s degree in Social Work required
- 1-2 years experience in a healthcare setting, preferred
- Current LICSW license through the Minnesota Board of Social Work
- BLS certification
Job Responsibilities:
- Assists patients and their families with concerns that accompany illness or inhibit recovery and rehabilitation.
- Interviews, assesses/screens, diagnoses, and treats patients in need of mental healthcare.
- Assists with making the necessary arrangements for leaving the hospital or clinic, such as nursing home placement or home health services, as backup to Medical Social Services.
- Initiates necessary referrals to appropriate community resources and provides crisis intervention as needed.
- Attends organizational committees and work groups as assigned by supervisor.
- Establishes and completes at least one quality improvement project each year.
- Promotes staff development by creating an environment of learning and rediscovery.
- Acts as a community liaison.
- Must have reliable transportation and a current driver’s license.
- Other duties as assigned.
