·Oversees and accountable for overall operations of Hebron Oaks and managing the partnership with Oakwood Preferred Hospice, with emphasis on empowering staff to be creative, offer input into the decision-making process, and be accountable for achieving goals as part of the larger Oakwood team.

·Ensures that team provides exceptionally high-quality services for the residents/families. Engages in prompt resolution of concerns that will result in resident/family satisfaction.Reviews services provided in view of market needs and future opportunities.

·With input from others, develops the facilities annual goals and objectives and assures that the action steps are implemented as scheduled. Ensures the goals and objectives are in alignment with the campus goals and overall Oakwood business goals.

·Meets and/or exceeds budget projections and ensuring that census remains at or exceeds projected numbers.

·Monitors medical director, health care providers and physician’s duties as related to requirements.

·Develops and manages the operational budget and capital budget for Hebron Oaks. Completes monthly written report to the Campus Executive Director, including a monthly financial analysis of revenue and expenditure variances.

·Monitors compliance with state and federal rules, regulations and codes pertaining to the operations of a skilled nursing facility.Works closely with state, federal and county agencies and develops healthy working relationships.

·Oversees the maintenance and protection of the building, equipment, and fixtures to ensure their continuous use and efficient operation. Determines that adequate sanitary conditions are maintained.Corrects unsatisfactory conditions that may arise in any phase of the operations.

·Maintains HIPAA compliance related to privacy and access to resident information and assists with electronic medical records (EMR) campus needs.

·Partners with Human Resources in performance management of team to ensure that the Human Resources policies and procedures are administered effectively and handled in a fair manner.

·Demonstrates and implements talent development of facility team by delivering career development, internal education; Develops, reviews, and revises job descriptions as needed and delivers performance evaluations for direct reports.

·Manages and oversees staffing model, creating a sound organizational plan of staffing to ensure the effective delivery of quality services and meeting PPD expectations.

·Serves as staff liaison for the Resident Council and Family Focus meetings in Hebron Oaks and advocating for resident needs.

·Maintains a positive image of Oakwood with hospitals, business partners, member churches and the community in general.

·Coordinates the development, implementation, and communication of policies and procedures for each department and facility.

·Provides reports to and at times may be asked to attend Board Council meetings and Resident Association committee meetings, as appropriate.

·Develops close supportive relationships with area hospitals for managing the admissions and discharge processes to achieve the payer mix objectives of Oakwood.

·Reports events/potential claims as needed to claims consultants in accordance with the Caring Communities’ Claims Manual and report incidents, allegations, and suspected occurrences to the Wisconsin Reporting System.

·Serves as an active member on campus and corporate teams including but not limited to: Campus Leadership Team, Safety Committee, etc.

·Facilitates monthly quality assurance meeting and quarterly Quality Assurance Assessment committee meeting with reviews of Quality Indicators regularly to achieve thehighest level of quality care and performance in these areas.

·Other duties as assigned.

 ESSENTIAL QUALIFICATIONS

  • Thorough knowledge of current principles and practices of skilled nursing and rehabilitative services administration.
  • Working knowledge of Federal and State regulations pertaining to skilled nursing facilities and rehabilitative services.
  • Proven history of commitment to providing exceptional clinical care of frail and aging adults. 
  • Demonstrates effective verbal and written communication skills and human relations skills.
  • Displays collaboration and partnership with the residents/families, staff, and the community in general.
  • Proven leadership fostering a team approach in a development culture.
  • Maintains and protects the confidentiality of information.
  • A proven track record of exercising independent judgment and making sound decisions.
  • Proactively plans, organizes, and prioritizes work schedule to ensure an effectively run facility.
  • Willingness to be flexible with hours in order to carry out the responsibilities of the position including outside of normal business hours.
  • Demonstrates creativity and innovation in establishing short/long term goals and objectives, and policies and procedures.
  • Engages, empowers and mentors the Hebron Oaks clinical team, therapy/rehabilitative services team, and health information team.
  • Works well within an atmosphere that promotes an environment of collaboration with other team members on the campus, between campuses (University Woods & Prairie Ridge) and with executive team members.

EXPERIENCE, EDUCATION AND/OR TRAINING 

  • Successful completion of a bachelor’s degree program in Health Care Administration or Business Administration or other fields related to the Health Care field required.
  • Minimum of three years administrative experience in a skilled nursing facility setting.
  • Must be currently licensed as Nursing Home Administrator in the State of Wisconsin.

PHYSICAL, MENTAL AND ENVIRONMENTAL REQUIREMENTS

  • Must be able to adapt to frequently changing work parameters.
  • Primarily works inside with frequent sitting, walking, standing bending and occasional lifting of supplies not in excess of 25 pounds.
  • Must understand and accept the possibility of exposure to environmental elements, such as inclement weather, dust, noise, chemicals and/or chemicals fumes, and infectious disease.
  • Normal physical mobility that includes movement from place to place.
  • Normal physical agility, that includes the ability to maneuver body while in place.
  • Normal physical strength, associated with this job, to handle routine materials and tools.
  • Normal dexterity of hands and fingers.
  • Normal coordination, including eye-hand, hand-foot.
