Education, Licenses, Certification, Skills and Experience Requirements:
- Minimum Education:
- Current, Professional Nurse Registration from the Wisconsin State Board of Nursing
- Current BLS(CPR) certification
- Preferred Work Experience:
- One to three years work experience
- Required Skills
- Ability to care for psychiatric patients in a clinic setting, including but not limited to rooming patients, obtaining vital signs, documenting chief complaint(s), allergies and other pertinent medical information
- Ability to use EMR for tasks such as medication reconciliation, accessing and updating medical/surgical history, family history, and substance use history.
- Ability to administer and document ordered medications following all guidelines as indicated by consent forms, etc.
- Driving Requirements:
- Valid Wisconsin driver's license and appropriate auto insurance coverage as required by North Central Health Care.
- Must have the use of a personal vehicle.
- Preferred Qualifications, Skills, Education, Licenses, Certifications and Requirements:
- Bachelor’s Degree in Nursing
- Previous experience working with individuals diagnosed with severe and persistent mental illness
- Crisis interventions skills
- Strong customer service, communication, and multitasking skills
Any combination of education and experience that provides equivalent knowledge, skills and abilities may be considered.
Essential Job Duties and Responsibilities:
- Reviews appointment schedules and maintains timely flow of
- Enters, obtains and ensures accuracy of information within the medical records, including vitals, weight, allergies, blood levels, physician orders, lab orders, HSRS, ROIs, consents, medication reconciliation, etc.
- Conduct appropriate screening and testing, as assigned, including PHQ-9, urine analysis, gene-site testing, blood sugar, etc.
- Manages psychiatry telephone line. Check messages, answering questions, advising patients with provider guidance, documenting call, gather additional information and make decisions within scope of practice or effect transition to appropriate staff.
- Communicates with patients, in multiple channels (including in person, electronic and telephonic) for appointment reminders, return phone calls, follow-up calls to check on patient progress, and notification of test
- Manage effective medication refill process, including verification of physician orders entered and verifying state drug monitoring program.
- Administers and documents ordered injections following guidelines with required form, e. consent forms, etc.
- Assist in office function e. distribute mail, faxes, requests charts, etc.
- Effects physician referrals to other programs or organizations.
- Prepares client communications, including return to work and any disability paperwork.
- Adheres to scheduled work hours with regular and predictable attendance.

- 0% Heavy Work: Frequent lifting, standing, walking, turning, twisting, bending, stooping, carrying, pushing, pulling, reaching, and grasping. Must be able to lift a minimum of 50 pounds unassisted
- 0% Very Heavy Work: Frequent lifting, standing, walking, turning, twisting, bending, stooping, carrying, pushing, pulling, reaching, and grasping. Must be able to lift a minimum of 100 pounds unassisted
