 Qualifications:

• Must be flexible, a team player, reliable, and have a positive attitude
• Must be a graduate of an APTA accredited BS, MS, or DPT level course in Physical Therapy
• Must have Kentucky Physical Therapist (PT) license
• Prior SNF experience a plus
