Position:  Director of Memory Care and other Units

The Palmettos of Bluffton is seeking a LPN Assisted Living and Memory Care Supervisor who is responsible for developing, coordinating, implementing, and overseeing the programs and services of the memory care unit.  The goals of the memory care program are to fulfill the basic psychological, intellectual, physical, social, and emotional and spiritual needs while promoting quality of life in a nurturing environment.

The qualified Licensed Nurse for this position should:

• Be well rounded, possessing patient-focused skilled nursing experience
• Possess superior communication skills
• Be dependable, flexible and passionate about senior care

Position Highlights: 

• Supervise the daily operation of the memory care unit and other units assigned.
• Daily rounds
• Supervision of the team
• Perform pre-move in assessments, as needed, in collaboration with Director of Nursing, and/or Sales and Marketing Director.
• Initiate, and update resident plan of care every six months, and revise care plans as needed.
• Coordinate and participate in care plan meetings.
• Strong clinical background preferred
• On Call 3pm-11pm
• Is responsible for maintaining clinical competency as evidenced by application of integrated nursing knowledge and skills, leadership, and communication skills.
• Utilizes the nursing process in assessment, planning and implementing care.
• Exhibits organizational ability related to workflow, prioritizing to meet the patient care needs
• Integrates current standards of practice as well as local, state, and federal regulations related to nursing services in the care of patients.
• Practices continuous quality improvement thinking and problem-solving skills.
• Must be caring, compassionate, good communication skills, have a positive attitude and be a team player
• Administer Medications per Physician orders.
• Documenting in patient's charts
• Assists in the preparation for admission of patients.
• Clinical and administrative tasks
• Assists in and accompany patients in admission, transfer and discharge procedures.
• Handling patient and family complaints
• Other duties as assigned 

Education/Licensure:

• Bachelor’s degree in related field with a minimum of (1) year experience specializing in Alzheimer’s and Dementia in a residential setting preferred.
• Licensed nurse
• Strong understanding of activities professional has completed a training course approved by the state.
