Purpose: 

The Patient Navigator works closely with the team registered nurses, medical providers, community partners, and collaborating social service agencies to support patients in successfully engaging appropriate clinical and support services.

In this role you will

  • Collaborate with patients, clients, staff, and community partners to identify and address the ways that systemic racism and other forms of compounding bias are root causes of disparities in clinical/mental health and homelessness in Black, Indigenous, and other communities of color and other communities that have experienced historical and structural oppression.
  • Support patients seeking assessment for substance use disorder and the appropriateness of MAT program enrollment in establishing care at Neighborcare Clinics.
  • Facilitate patient connection to and coordination amongst needed clinical and community resources including those related to transitions in care.
  • Provide support including accompanying patient to appointments.
  • Collect patient data and enter into Electronic Health Record (EHR) and/or tracking tool.
  • Monitor and report patient progress via tracking tool and monthly reports.
  • Lead outreach and engagement activities with community partners.

  • Familiarity working with vulnerable populations including those experiencing substance use and/or mental health disorders, homelessness, and those with limited English Language proficiency.

Required Qualifications

  • AA Degree
  • 1-3 years of of relevant work experience

Preferred Qualifications

  • One (1) year of experience in a healthcare setting
  • One (1) year of experience in a customer service setting
  • Bilingual skills

Compensation 

The wage range for this position is minimum $19.97 to $27.25 maximum.

Compensation will be determined based on years of relevant experience.
