The Registered Nurse Case Manager is responsible for providing direct patient care, coordination of overall home care services and documentation of care for agency clients in accordance with Quality Standards and under the guidance of a Clinical Supervisor.

Home visits are conducted Monday-Friday, 8a-4:30p, with one weekend per month required. There is No On-call required. Although this position primarily serves Jefferson County, visits to surrounding counties may be required as needed.

Performance Responsibilities and Standards:

  1. Ensure quality and safe delivery of home health services.
    1. Case manage a diverse client caseload.
    2. Develop a plan of care based on client assessment and OASIS evaluation.
    3. Develop a plan of care using interdisciplinary approach.
    4. Ensure that all documentation meets quality standards:
      1. Completes all OASIS evaluations.
      2. Documentation reflects nursing care and interactions.
      3. Documentation is clear and concise.
      4. Documentation is completed in timely manner
    5. Understand and follow nurse code of ethics and standards of practice.
    6. Coordinate with internal and external resources to meet patient needs.
    7. Maintain established productivity expectation
    8. Communicate client care issues to interdisciplinary team.
  2. Responsible for coordination of RN’s and LPN’s.
    1. Provide guidance and education in client care to LPN’s.
    2. Work in conjunction with Clinical Supervisor and Patient Care Coordinator (PCC) to efficiently manage patients using a team approach while appropriately utilizing LPN’s.
  3. Responsible for supervision of paraprofessional staff relevant to patient care.
    1. Supervise home health aides in performing designated tasks related to patient care and per care plan.
    2. Communicate appropriate information to home health aide department.
  4. Comply with Nascentia policies and procedures and quality initiatives.
    1. Maintain confidentiality.
    2. Demonstrate knowledge and use of infection control practices.
    3. Demonstrate knowledge of federal, state and local government rules and regulations.
    4. Demonstrate knowledge and compliance with agency policies and procedures.
    5. Adhere to quality improvement standards.
    6. Keep personnel requirements up-to-date; i.e., health requirements, licensure requirements, and other Nascentia requirements.
    7. Demonstrate skills competency through periodic evaluations.
  5. Participate in personal growth and development.
    1. Participate in educational in-services and meetings.
    2. Maintain and update clinical knowledge on an ongoing basis.
  6. Present a professional image to internal and external customers in compliance with Nascentia’s dress code.

Job Qualifications:

  1. Current New York State registered professional nurse license.
  2. Degree from an accredited school of nursing or college.
  3. Current and valid driver’s license and auto insurance, and a vehicle which can be used for work.
  4. Self-directed and organized with the ability to work independently.
  5. Good oral and written communication skills.

Special Skills:

  • Knowledge and skill in use of Microsoft office
  • Ability to learn organizations software system to perform job duties.

Physical Requirements:

1. Speaking/visual/hearing ability sufficient to comprehend and express written/verbal communication.

2. Ability to perform tasks involving physical activity including:

Pushing/pulling - Typical weight 50 pounds / Maximum weight 150 pounds

Lifting/ carrying - Typical weight 25 pounds / Maximum weight 40 pounds

3. Patient transfers, extensive bending, standing, kneeling, walking in various weather conditions e.g. snow, rain, sleet, and going up and down stairs.

4. Ability to drive in all weather conditions throughout the working year.
