The Home Care Coordinator, RN/MSW (Registered Nurse OR MSW) is responsible for developing home care plans for referred Hospital, Rehab, or Skilled Nursing Facility patients. This position involves visiting facilities daily as required to assess and screen patients for appropriateness of home care services.
Performance Responsibilities and Standards
1. Promote continuity of care for referred clients (including but not limited to):
- Determine client/family home care needs upon referral for home care.
- Assist in the development of a home care plan including the arrangement of needed services, supplies, and appliances.
- Review home care policies and procedures with clients/families, physicians, agency care providers, insurance providers and/or service & equipment vendors.
- Act as a liaison between Nascentia Health and hospitals/rehabs/SNF.
2. Promote quality assurance (QA) as it relates to home care planning (including but not limited to):
- Document assessment and home care eligibility per Nascentia Health policies and procedures.
- Act as a resource and/or consultant to hospital or SNF staff regarding home health services.
- Participate in quality improvement activities relating to home care planning and coordination.
- Maintain accurate up-to-date statistics regarding home care referrals.
- Participate in team conferences between client/family, physician, hospital staff and home care agency staff.
- Work closely with Customer Service Center Team Lead (RN position) for support in maintaining referral expectations within the agency's daily and future capacity projections.
3. Participate on agency committees
4. Present teaching and education to patients/family, caregivers, and providers about Nascentia Health Homecare, our mission and all services available.
5. Perform other duties as assigned by direct supervisor and/or department director.
Requirements
1. Current license to practice as a Registered Professional Nurse/ Medical Social Worker in New York State or degree in a health-related field.
2. Minimum one (1) year home health care experience preferred.
3. Current New York State driver's license and automobile insurance required.
4. Excellent verbal communication skills required.
Physical Requirements
- Ability to sit or stand at a desk 90% of the day
- Visual/hearing ability sufficient to comprehend written or verbal communication
- Ability to express self verbally and in writing
- Ability to use telephone and communicate over the phone
- Frequently uses a laptop computer to enter orders and obtain product information
- Bloodborne Pathogens Exposure Category III - Employee who rarely has contact with blood and body fluids
