Description
Social Worker Care Management, Full-time, 80 hours bi-weekly
Works in collaboration with the care management team and other healthcare providers to meet the psycho-social care needs of patients through their acute episode of care. Develops a discharge plan and strategy that addresses the patient needs through the continuum of care post-discharge. This professional collaborates with the healthcare team to achieve optimal clinical, financial, and patient satisfaction outcomes. This professional works in an interdisciplinary environment and is tasked to assess patients for care coordination needs, facilitate the physician plan of care, implement and coordinate care progression, and monitor and evaluate options and services to meet an individual’s health needs. This professional accomplishes those tasks through effective communication, collaboration, knowledge of evidence-based care, coordination of community resources, and other interventions requisite to efficient clinical care progression for optimal patient health outcomes.
Minimum Qualifications:
Education - Bachelor’s degree in Social Work required. Master’s Degree preferred.
Experience - Requires at least one year of acute care experience with LBSW OR Master's Degree with relevant clinical rotation experience.
Licensure/Certification - Minimally licensed at LBSW in the State of Maryland. Must obtain care/case management certification within 5 years from accrediting body.
Knowledge/Skills/Abilities - Multi-tasking required, strong communication skills required, knowledge of evidenced based practices preferred, knowledge of disease processes preferred, knowledge of regulatory requirements, computer skills required, electronic medical record navigation strongly preferred, knowledge of the revenue cycle, knowledge of community resources and medical staff community is of strong benefit. Networking with community partners required.
