We are looking for an LPN to join our Internal Medicine Team as a Care Coordinator! 

What You'll Do:

• Support 3 providers

In this role, you will have the opportunity to help support patients by collaborating with patients to:

• Set healthcare goals
• Complete advance care plans 
• Track providers quality goals

What You'll Bring:

• Completion of post secondary program in Medical Assistant OR
• Nursing degree
• Current Ohio LPN license
• Knowledge of Microsoft Office
• Preferred:
  • Experience as a physician offices 
  • BLS/CPR certified preferred.
  • Proficient with technology required for data tracking and reporting
