Description

Clinical/Technical/Case Management: Receives report from primary Registered Nurse.  Collects and prints nursing assessment data such as medications, labs, treatments, and multidisciplinary assessments for communication to the physician, patient/family, and/or nursing staff.  Coordinates the Provider’s assessment, medical plan of care, and management of the patient.  Recognizes, interprets, documents and communicates information necessary for quality patient care and related patient information.  Maintains confidentiality at all times to protect patient’s privacy and maintains Health Insurance Portability and Accountability Act (HIPAA) privacy and security regulations. Provides, delegates, and coordinates patient care: assists the patient, physicians, nursing and other allied health care providers in developing care plans and or problem lists consistent with evidence based practices.  Demonstrates knowledge of the nursing process (e.g. assessment, planning, implementation, evaluation, etc.) while considering age, growth, development and specific needs of patient population to provide nursing care.  Involves family members and significant others in process following HIPAA guidelines.  Makes visual observations, hears sounds and tones, identifies odors, uses interview techniques to assess the patients age, physical status, psychosocial status, economic status, spiritual needs, language barriers, mental states and level of knowledge and understanding.  Rapidly plans and evaluates patient care on assessment findings.  Develops and maintains a list of patient problems or nursing diagnosis and desired outcomes.  Implements plan of care including the provision of education appropriate to the patient’s age, level of comprehension, physical needs, psychosocial needs and social cultural status on an individual basis. Rounds with the hospitalist provider for the purpose of care coordination, collecting data through chart review, interview, and observation. Perform procedures consistent with hospital policy/procedures, including: complete dressing changes, removal of sutures, assist provider with treatments and examinations. Assists in the medical plan for management of the patient by performing interventions within the Registered Nurse scope of practice as defined by the Ohio Nurse Practice Act and Rules, Regulations, and Policies of the Ohio Board of Nursing. Records verbal and telephone orders, from the provider, on the medical record consistent with hospital policy. This includes reading back the verbal and telephone orders and following all current policies related to this process. Record data for discharge summary with additional notation, completion, and countersignature by supervising physician including dictation of discharge summary only after review of data with supervising physician. Coordinate patient’s discharge, including but not limited to:  medication assistance, durable medical equipment assistance, Home Health Care placement, placement in extended care facilities, and scheduling the patient for follow-up appointments/testing. Participate with the supervising physician in providing /family education regarding: diet, medications, disease information, exercise/activity, discharge planning and follow-up care. Plays a large role in orienting new providers to the hospital computer systems and environment of care. Learns new information to provide for a safe patient care environment. Keeps current with practice for nursing and professional development. Rounds with Hospitalist Physicians daily. Responds to patient needs outside normal scheduled work hours while on call for the organization; Responds to disaster alerts; documents all on call activity. Maintains accurate data collection for direct patient care and indirect care. Uses MIDAS + and Paragon computer systems for documenting information. Documentation is appropriate on hospital forms and completed in accepted established time frames. Reviews and documents advance directives according to policy. Refrains from using unaccepted abbreviations in written documentation. Enters and retrieves information from computer (Paragon, Midas +, Laser Arc). Critical Thinking: Actively looks for and creates opportunities to improve the department, staff and personal development. Develops and demonstrates knowledge of current developments in field to maintain professional competency. Utilizes hospital and departmental policies and procedures in patient care delivery. Follows the Ohio Nurse Practice Act. Communication: Clarifies and answers questions for patients/families and nursing staff about the provider’s medical plan of care. Communicates in order to educate patients/ family; provides kindness and consideration in meeting the emotional needs of patients; confers with physicians. Provides excellent customer service, facilitates quality care delivery and fosters an atmosphere of understanding cultural diversity. Communicates and assists physicians as indicated. Maintains appropriate written, verbal and telephone communications. All interactions are conducted in a professional manner. Demonstrates a positive attitude. Resolves conflict through one on one negotiation or with the assistance of Director or designee. Demonstrates the philosophy of team concept; participates in unit projects, attends committees as assigned, and attends monthly staff meetings; provides department report of committee activity. Communicates dissatisfaction with issues to Director; actively contributes to the solution of problems and refrains from promoting dissatisfaction among co-workers. Unit Financial Accountability: Maintains appropriateness of supplies. Is accountable for productivity and management of time. Understands hospital goals for financial viability. 
Education: Maintains professional competency; actively contributes to the solution of problems; deals with problems involving several variables within familiar context. Responsible and accountable for maintaining own state board required CEU’s per licensing board requirements. HIPAA privacy regulation and related procedures. Centers for Medicare and Medicaid (CMS) updates on benefit coverage and acute care policies. Reviews Case management literature as distributed by director. Maintains bi-annual BLS certification. Attends all mandatory education and in-services (i.e., team training, safety, infection control, etc.); completes mandatory health requirements. Employee performs within the prescribed limits of the hospital’s and department’s Ethics and Compliance program and is responsible to detect, observe and report compliance variances to their immediate supervisor, or upward through the chain of command, the Compliance Officer, or the hospital hotline.

Requirements
Completion of an accredited school of nursing with current active registration in the State of Ohio as an RN in good standing. Registered Nursing degree licensed in the State of Ohio; completes the required 24 contact hours of approved CE during each two year renewal period consistent with the Ohio Board of Nursing requirements. Maintains continuous certification in American Heart Association’s BLS.
