Certified Hospice Aide - Casual/Per Diem

Lumina Hospice & Palliative Care in Corvallis, Oregon is seeking a casual (per diem/as needed) Certified Hospice Aide to join our team. As a Certified Hospice Aide you’ll get the opportunity to work with programs and services designed to honor choice and dignity, while providing comprehensive patient and family care. As the Certified Hospice Aide with Lumina Hospice & Palliative Care, you play an important role in our community and for our clients.

Your responsibilities will be to:
Provides care consistent with the individual patient's Plan of Care and as directed by the Primary Nurse. Duties may include any or all the following:

  • Provide patients with personal care in accordance with the individualized plan of care and visit frequency assigned.
  • Assist patients in bathing, care of mouth, skin, and hair, and assist patients in and out of bed and with ambulation.
  • Perform turns of patients in bed and assist in positioning patients for comfort as well as changing bed linens as needed.
  • Report changes in patient's condition and needs.
  • Maximize the comfort, safety and health of patients and families consistent with hospice philosophy and the organization’s policies and procedures
  • Maintain up to date patient records in accordance with policies and procedures so plans, actions, goals and problems are accurately and clearly stated

Performs housekeeping tasks as assigned to maintain hygiene and comfort of the patient's environment:

  • Completes appropriate records in a timely way.
  • Performs all duties in a team environment, offering and receiving assistance as fluctuating workloads and staff absences indicate.
  • Completes a minimum of 12 hours of in-service education and training programs annually to comply with Medicare regulations.
  • Performs other duties as assigned by the Primary Nurse or Director of Nursing.
  • Contributes to communication, positive work environment, and team effort
  • Serves as assigned on agency-wide initiatives and/or committees.
  • Participates in QAPI activities.
  • Understands and adheres to Lumina Hospice & Palliative Care Privacy Policy; Lumina Hospice & Palliative Care Abuse of Vulnerable Persons Policy; and all policies and procedures contained in the Lumina Hospice & Palliative Care Employee Handbook.

Qualifications:

  • Requires a minimum of one year of prior CNA, Hospice Aide, or equivalent work experience.

  • Highschool diploma or equivalent.

  • Current certificate of practice as a Certified Nursing Assistant (CNA) in the state of Oregon; must take and pass the Hospice Aide exam.

  • Must have Basic Life Support CPR card for Health Care Provider. Valid card must be obtained prior to patient contact and provided within two weeks of hire.

  • Must have a valid driver’s license and acceptable driving record and have access to a reliable automobile to travel to and from patient homes.

  • Demonstrated skill in communication; patient documentation requirements, and service coordination with the ability to work successfully with patients and family members in aspects of terminal care.

  • Must have solid skills in using electronic tools such as email, internet, keyboarding and be able to learn medical software to use for accurate and timely charting.

  • Must be reliable and have the ability to work independently and as a member of a team in delivering assistance within scope of practice.

  • Ability to follow oral and written instructions.

  • Ability to make meaningful observations and write brief, accurate reports.

  • Reliable in performing duties in a timely fashion.

  • Must be physically and mentally capable of performing all essential functions required of the position.

  • Position requires ability to lift up to 40 pounds without assistance and to roll by pushing and pulling up to a 200 pound person who is able to assist or 150 pound person who is not able to assist.

  • Bi-lingual, Spanish speaking ability is helpful.

  • Schedule: Casual/on-call as needed. Usual hours fall within the 8-5, Monday through Friday work week although occasional other hours may be required to meet patient needs and fluctuating workload.

    *Preferred applicant will be asked to pass a background check and drug screen following a job offer.
    **New employees are requires to complete Tuberculosis screening and provide proof of Hepatitis B vaccination or signed declination.
    ***Must be able to show proof of COVID-19 vaccination.
