Kettering Health Dayton
- Kettering Health Dayton, formerly Grandview Medical Center, has been providing care to Dayton and surrounding communities for over 90 years.
- Over 20 specialties, including orthopedics, heart and vascular, surgery, and trauma services.
- KH Dayton expanded their Emergency Department as well as their heart & vascular services to include an additional catheterization lab and electrophysiology lab.
- Partners with Gem City Market to provide fresh fruits and vegetables every Tuesday within the hospital.
- One of the largest osteopathic teaching hospitals in the United States.

Responsibilities & Requirements
KEY JOB RESPONSIBILITIES:
- Establish practice by interacting with patients, prescribers, nurses and other healthcare professionals, as a member of an interdisciplinary team in physician practices to provide pharmacotherapeutics recommendations and monitoring for optimal medication use.
- Work closely with physicians and health care teams, while providing direct patient care under a collaborative drug therapy management agreement (CDTM),
- Meet with patients to provide extensive drug and disease state education with a focus on patients at high risk for hospitalization or poor health outcome.
- Work closely with patients to overcome medication access and adherence barriers
- Serve as a drug information resource for the other providers and office staff, answering medication therapy questions
- Monitor for drug-therapy-related adverse events by performing/ordering the appropriate clinical or laboratory tests
- Engage in population health projects
- Provide education for pharmacy staff, students, nursing and other healthcare professionals.
- Serve as a preceptor for pharmacy students and pharmacy residents.
- Participates in strategic planning, business development, and public relations
REQUIREMENTS:
Education:
Pharmacy degree from accredaited college of pharmacy.
PGY1
PGY2 Residency in Ambulatory Care Pharmacy preferred
Skills and Experience:
- Must have exceptional patient communications and be able to modify patient health behavoir
- Must be able to share complete and accurate information with physicians and other health professionals
- Must have strong background in primary careMust understand the financing of population health
- Must be competent and proficient to use computers and applications, understand and utilize the electronic medical system, and related equipment within the pharmacy.
- Must be able to prioritize work to assure that most emergent situations are handled first, and medication safety standards are practiced always to provide excellent and safe patient care.
- Must be able to collaborate and communicate professionally with the staff, patient family members, patients, physicians, outpatient pharmacies, and other healthcare personnel both effectively and efficiently daily.
- Must possess strong oral and written communication and interpersonal skills.
- Must uphold and complies with Corporate HIPAA regulations, all Institutional and KHN policies and procedures, and Pharmacy Departmental policies and procedures. Must complete all mandatory education and competency required annually. Must comply with KHN Standards of Behavior.
Licensure, Certifications, Registrations:
Must register with the Ohio State Board of Pharmacy as a Pharmacist and maintain active status during their employment at Kettering Health
Board Certified – Obtain BCACP certifificatin within two years of starting in role
