Position Overview
As Senior Clinical Case Manager, reporting directly to the Director of Supportive Housing, you will provide supervision over the case management staff as well as the interns. Additionally, you’ll also provide case management services to the residents of Woodycrest Supportive Housing, who often have substance use and/or mental health disorders. Services will include individual counseling, benefits coordination, service linkages to community resources, intensive care referrals, assessment of employability, and increasing housing stability.
Specific responsibilities will include:

Managing a tenant caseload not to exceed 15 households at any point in time.

Providing bi-weekly supervision to case managers and student interns.

Conducting regular audits of charts and files

Conducting assessments, formulating individualized service plans, and providing referrals and ongoing support to assist tenants in developing skills and achieving personal goals associated with housing stability and well-being.

Serving as the foundation of the supportive services made available to tenants, such as connecting them to community-based services and resources that are aligned with their ISP.

Facilitating a range of support groups and workshops designed to help tenants live as independently as possible.

Serving as a liaison with medical, mental health and substance abuse providers to ensure a level of care that the consumers deserve.

Aiding with educational and employment linkages.

Helping tenants overcome barriers that may affect the timely submission of monthly rent.

Working with property management to support tenants in all aspects of maintaining permanent housing

Advocating for and with residents in all aspects of their lives and offering (but not imposing) assistance.

Participating in weekly individual supervision with the Director of Supportive Housing-Bronx Region, departmental staff meetings, interdepartmental team meetings with property management staff, and team case conferences.

Utilizing the AWARDS electronic case management system to document case information and to maintain electronic case records, maintaining physical case records as required to comply with regulations and policies set forth by HELP USA and DOHMH, and collecting and submitting weekly and/or monthly performance reports and statistical information as requested by DOHMH.
Requirements

Master of Social Work or related field required.

A minimum of 3 plus years of experience with case management and supervision, service coordination, assessment, counseling, and crisis intervention.

Experience and knowledge of NYC benefits programs, rental assistance resources, and housing subsidy programs preferred.

Strong oral, written, and negotiation skills.

Strong organizational and documentation skills.

Ability to work both independently and as part of a team.

Ability to take the initiative, be dependable and resourceful, and pay keen attention to detail.

Knowledge and understanding of family systems approach to practice and experience with Motivational Interviewing and other best practices preferred.

Computer literacy, particularly with Microsoft Office applications.

Valid NYS driver’s license a plus.

Availability to provide evening and weekend coverage as needed.
Compensation: $31.25/hour
