Position Title: Referral Coordinator

**Hours are 9am - 6pm with occasional weekends*

Overview: The Referral Coordinator receives and responds to community, physician, patient and family requests for assistance. This position records, processes referral, admission information, and documents in patient information database. The Referral Coordinator schedules registration/admission visits and prepares preadmission and admission paperwork as well as act as a liaison between patients/families, referral sources, physicians and admission staff. They will also assist in distribution of admission information and report to primary teams and physicians. The Referral Coordinator assists in processing daily reports reflecting census and referral admission status.

You Have:

• High School or Equivalent,
• 1-2 years of experience in related healthcare position preferred.
• Customer service experience required.
• Valid Florida drivers license and insurance required.

• Ability to work non-judgmentally with persons of varied cultural, religious and socioeconomic backgrounds and lifestyles.

Location:  Orange Park Service Area

The Orange Park Care Center is Haven’s fifth free-standing care center serving north Florida is a state-of-the-art facility that allows us to expand our services to better meet the end-of-life and palliative care needs of patients and families in the area.  The care center is located at 745 Blanding Blvd, Orange Park.
