Position Title: After Hours Response Team – LPN

Overview: Haven’s After Hours Response Team nurse responds to calls from patients, families, caregivers, and referral sources. The nurse assesses, utilizes critical thinking and gives advice or delivers care to patients utilizing the nursing process of assessment, planning, interventions, implementation, and evaluation; and effectively interacts with patients, family members, and other interdisciplinary team members while maintaining standards of professional nursing and clinical competency.

You Have: • Current Florida LPN License required • IV Certification Required • Minimum of 2-3 years nursing experience required, preferably in oncology, palliative care, geriatrics, or home health care. Alignment with the Haven mission, culture and values of collaboration, accountability, respectfulness, ethical behavior and service-driven. • Ability to work non-judgmentally with persons of varied cultural, religious and socioeconomic backgrounds and lifestyles.
