Position Title: Admissions Patient Care Manager
**Monday - Friday, 8am – 5pm, with on-call one week per month.**
Supervises and coordinates hospice admissions and patient care to ensure timely access to care and quality hospice care delivery twenty-four (24) hours a day, seven (7) days a week for Haven’s hospice program across all settings including inpatient care, home care, facilities, and hospitals. Coordinates admissions of patients to hospice with demonstrated knowledge of hospice eligibility, criteria, certification requirements and changes in hospice level of care.

You Have:
- Education – Bachelor’s in Nursing or a related field of Healthcare required, Master’s preferred. Experience will be considered in lieu of degree.
- Licensure & Certifications- Current Florida RN license without restrictions.
- Requires a current Cardiopulmonary Resuscitation (CPR) certification from the American Heart Association. Hospice and Palliative Care Certification preferred.
- Florida driver’s license and auto insurance required.
- Experience – 1 year of managerial experience required and Hospice experience strongly preferred.
- Energetic team player with ability to "switch gears" as workload requires.
- Position requires excellent listening, communication and organizational skills, accurate entry of documentation into patient information database, and ability to handle multiple tasks/calls. - Ability to work non-judgmentally with persons of varied cultural, religious and socioeconomic backgrounds and lifestyles.
