General Summary: Provides direct patient care to patients and assists doctors as needed. Provides leadership, direction, and support to medical assistants on patient related issues. Assists fellow employees in providing the highest quality, personalized patient care and maintains a positive working relationship with the health care team. Supports the Gonzaba coordinated care model by working with other GMG providers and support departments/functions to ensure a patient-centered approach is provided at all times.
Supervisory Responsibilities: This position has no supervisory responsibilities.
General Requirements: All duties performed will be done accurately and in a timely manner.
- Provides leadership, direction, and support to medical assistance and LVN's on the issues of outpatient care, medications, or scheduling.
- Assist different teams as necessary including performing the duties of other personnel as needed.
- Participates in educational programs as needed.
- May need to travel to support patient care at another GMG location.
- At times the LVN will need to float to other pods (in the clinic or another location), during these times all pod responsibilities should be completed including but not limited to triage, discharge, phone notes, refills, call confirmations, etc.
- Exercise tact and courtesy when dealing with patients, visitors, providers, and co-workers.
- Must always adhere to customer service expectations including in-person and virtual (via telephone, or telehealth applications) communication.
- Maintains strict confidentiality.

Essential Job Responsibilities:
- Engages patients’ regarding healthcare education, medication admin, medication administration, triages walk-in patients and Triage calls when needed.
- Maintains cabinet with medical samples: categorization, expiration dates, etc.
- Participates in and aids during code blue situations as directed by the code blue team leader.
- Provides patient care in an out-patient clinical setting such as starting IVs, foley catheterization, and assist with other patient procedures as requested by doctors within the scope of the practice as defined by the Texas Board of Nursing.
- Reports and accurately documents clinical findings to provider in charge.
- Performs patient discharge and participates in patient education utilizing provider guidelines and/or patient information handouts. Ensures patient understanding and documents thoroughly.
- Assists in providing quality health care by accessing the electronic medical record to obtain/provide pertinent information. Reports appropriate information to the provider and documents appropriately.
- Furthers patient care by communicating effectively with providers, coordinating tests, procedures, surgeries, and hospitalizations and providing patients with community resources as needed.
- Remains aligned with the departmental goals which involves understanding the importance of conducting RMH visits, perform HEDIS measures, and participating providing quality care by utilizing the tools and reports provided to help reach departmental/organizational goals.
- Maintains assigned work area with supplies and equipment. Cleans, stocks, and maintains exam rooms, procedure rooms, instruments, and equipment.
- Follows universal precaution standards about patients, procedures, handling specimens, cleaning and disinfecting instruments and exam rooms as taught and retrained through annual compliance training.
- Transports patients safely from the following positions: sitting to standing or laying down, from standing to sitting or laying down, from laying down to standing or sitting, and assist to the bathroom, into/out of wheelchairs, and pushing/pulling patients in wheelchairs.
- Must always keep the patients’ safety a priority, which will involve preventing potential falls as needed.
- Assumes responsibility for maintaining clinical competencies according to Gonzaba Medical Group policy.
- Ensures customer service is always maintained at the highest level.
Education and Training: Graduation from an approved practical nursing program. Must be state-licensed practical nurse with one year of LVN and clinic experience preferred. Must be CPR and AED certified. Must keep all licenses and certifications current and in good standing as initial and continued employment is contingent up these credentials. Must have knowledge of medical terminology, anatomy, physiology. Familiarity with health care systems, regulations, policies, and functions. Must have a good understanding of documentation standards.
Experience: Experience in a clinic, doctor’s office or hospital with a minimum of one-year experience preferred.
Other Requirements: Computer Skills: Skilled in use of computer/EMR systems. Knowledge of Word processing software, spreadsheet software, Internet, and database software.
Work Environment: Clinic environment. Exposure to communicable diseases, bodily fluids, toxic substances, ionizing radiation, medicinal preparations, and other conditions common to a clinic environment.
Mental / Physical Requirements: Requires manual dexterity, sitting, standing, stooping, reaching, kneeling, crouching, bending, walking, lifting up to 40 lbs. without assistance. Close vision and ability to adjust focus. Must be able to work efficiently under pressure.
