Qualifications:

  • Education: Bachelor of Science in Nursing (BSN) preferred.
  • Licensure: Current and unencumbered Registered Nurse (RN) license in the state of practice.
  • Experience: A minimum of two years' experience in a management or supervisory role within a home care or home health setting is highly preferred.
  • Technical Skills: Strong knowledge of accepted professional standards, Medicare Conditions of Participation, and federal, state, and local regulatory requirements.
  • Soft Skills: Exceptional leadership, innovation, mentorship, communication, and interpersonal skills. Ability to serve as a key liaison between patients, families, and staff.

Key Responsibilities:

  • Lead and manage the delivery of patient care services, ensuring compliance with all regulatory standards.
  • Provide supervision, mentorship, and support to nursing staff to promote professional development and optimal patient outcomes.
  • Collaborate with interdisciplinary teams to create and implement personalized patient care plans.
  • Ensure precise and timely documentation of patient care and progress.
  • Conduct regular audits of patient records and care plans for quality assurance.
  • Serve as a communication link between patients, families, and healthcare providers to ensure seamless care coordination.
  • Engage in continuous quality improvement initiatives and professional development activities.
