Functional Pathways, a national provider of rehabilitation services, is hiring for a Restorative Nurse Manager to join our dynamic therapy team managing the restorative nursing department at Ben Atchley Tennessee State Veterans Home in Knoxville, Tennessee.

The Full-time Restorative Nurse Manager will educate restorative staff on providing resident-centered care, utilizing the Nursing Plan of Care and will report directly to the Director of Rehabilitation, under the supervision of the Director of Nursing.

Responsibilities:

1. Coordinates restorative nursing program throughout the facility.
2. Works with nursing administration and FP Clinical Reimbursement Specialist to
develop resident centered, individualized restorative programs.
3. Evaluates functional assessments upon admission and quarterly assessments
completed by nurses, for the development of individualized programs.
4. Works closely with therapists, ensuring that specific recommendations are
carried out following discharge from therapy. The restorative nurse
coordinator assigns the care to RNAs or unit CNAs and provides them with
education for the safe and knowledgeable delivery of the restorative programs.
5. Works with IDT to develop restorative plans of care, making sure all
restorative needs and care areas are addressed.
6. Ensures that all restorative nursing measures are reflected in the care plan. Care
plans are written the day of program initiation and updated monthly with auditing
procedures.
7. Keeps an up to date master log of all residents on a restorative nursing program,
the types of programs, and their initiation dates.
8. Keeps current the daily documentation flow sheets or EMR tasks for each resident
participating in restorative nursing programs. Provides guidance to restorative
Nursing Assistants regarding their daily documentation. Keeps Kardex up to date
with all active restorative programs.
9. Coordinates the initiation of toileting trackers for all new admits and residents
with change of condition. Assesses the 3-day trackers for patterns and develops
individual toileting programs for all qualifying residents.
10. Visually audits each restorative program being delivered no less than monthly.
Conduct resident interviews no less than monthly. Completes at least one
assessment note per month for residents on restorative programs with the results
of audits and resident interview responses included.
11. Attends care planning conferences and makes recommendations and/or reports
on progress of restorative nursing measures.
12. Educates RNAs, CNAs and charge nurses on all new programs initiated.
13. Maintaining signature logs of all educations both general to all nursing staff and
specific with each program initiated-and maintain record of the
education/trainings.
14. Monitors the effectiveness of the restorative nursing program by completing
quality assurance audits per policy.
15. Maintains a checklist of restorative nursing program competencies for each
staff member, ensuring that all staff are properly trained/in-serviced in
restorative nursing measures on an ongoing basis.
16. Complete a weekly report and attend weekly meeting with Functional Pathways
Clinical Reimbursement Specialist


Qualifications:

• Graduate of an accredited school of nursing
• Current license
• Two years’ experience in Geriatric care; Management/Restorative experience
preferred
• Understands the philosophy, principles, and techniques of restorative nursing
• Knowledge of long-term care, OBRA, and rehabilitation care
• Ability to coordinate Restorative Nursing Program and provide guidance and
training to other nurses, RNAs, and CNAs regarding restorative procedures
