Responsibilities and Duties include, but are not limited to:

Attend required meetings and trainings as scheduled.
- Assist with new admission chart audits to ensure documentation accuracy and compliance.

Support residents in completing the Medicaid application process.

Ensure timely follow-up on Medicaid pending applications, documentation, and status updates with appropriate state agencies.

Provide support to the rehab unit, assisting with admissions and discharges.

Coordinate delivery of services for Managed Care and Medicare residents in collaboration with the interdisciplinary team.

Communicate consistently with insurance and managed care case managers to facilitate certifications and stay approvals.

Participate in the orientation process for new residents and their families to ensure a smooth transition.

Schedule and coordinate care conferences involving residents, families, and facility staff.

Contribute to discharge planning, including the development and implementation of individualized social care plans and assessments.

Facilitate resident discharges, including arranging for necessary post-discharge services.

Assist residents and families in accessing community resources for discharge and aftercare support.

Help develop, implement, and regularly update the resident’s written discharge plan.

Complete the social services components of the MDS (Minimum Data Set) accurately and within required timeframes.

Collaborate closely with the Social Worker, Clinical Team, Business Office Manager, and other departments to ensure comprehensive care and resident advocacy.
GENERAL REQUIREMENTS
- Valid and active state-issued RN License
- Nursing Degree from an accredited college or University
- 1 year of experience as a supervisor in a hospital, nursing care facility, or other related health care facility.
