Responsibilities:

  • Coordinate intakes and discharges in adherence with policies and procedures.
  • Monitor utilization of service hours to avoid over or under utilization.
  • Provide direct care and support to residents as needed.
  • Oversee day-to-day operations of the group home program.
  • Collaborate with healthcare professionals, families, and external service providers.
  • This position is on-call and will serve as a part of an on-call rotation.
  • Advocate for the rights and well-being of residents.

Location, Scheduled & Pay:

  • We have various locations across the Twin Cities metro with various schedules.
  • This position is typically Monday-Friday, standard business hours.
  • Starting salary of $50k annually and goes up depending on education and experience with a $750 hiring bonus

What qualifications are required?

  • High School Diploma or GED equivalent preferred.
  • Bachelor’s degree in a human service related field preferred.
  • Must have two years of professional experience working with individuals with special needs. One year of supervisory and program coordination experience preferred. Excellent attention to detail and organizational skills.
  • Valid driver’s license is required.
