Fraser is seeking a Targeted Mental Health Case Manager! In this role, you will work with clients who range in age from infancy up to 18 years and their families. Clients are primarily internal referrals who are receiving other Fraser services such as Occupational Therapy, Pediatric Therapy, Speech Therapy and more.

Requirements:
- Must be qualified in at least one of the following ways:
- A bachelor’s degree or higher in one of the behavioral sciences or related fields from an accredited college or university
- Master's or other graduate degree in one of the mental health professional disciplines from an accredited college/university.
- If degree is in social work, license is also required.
- PREFERRED: 2,000+ Clinical/Supervised hours in delivery of clinical services.
- Previous experience working with individuals ages 0-21 required.
- Valid Minnesota Driver’s License with acceptable record required.
- Ability to pass MN DHS background study.

Salary Range: Starting at $44,000 annually + $750 Hiring bonus! Salaries can increase with experience and a completed master's degree in a field related to behavioral science.
Responsibilities:
- Collaboratively develop Individual Family Community Support Plans (IFCSPs) based on the Diagnostic and Functional Assessments.
- Provide resources in the following areas: mental health, social, educational, health, recreational, and more depending on the client and family needs.
- Collaborate with internal and external partners in order to support clients and their families.
- Meet with each client once per month (at minimum) and check in on progress and goals.
