Fraser is seeking a Care Coordinator responsible for supporting same day access and supporting individuals seeking services through Fraser. This position will work with individuals and their care team, as well as in partnership with mental health providers on the Care Coordination team, to successfully complete the initial assessment process and connect with supportive resources.
Available Locations and Schedules:
Location: Fraser Bloomington/Virtual Office & Community-Based
Schedule: Monday-Friday, standard business hours. This is a Full Time position
Pay Rate -Starting at $43,000 annually

Responsibilities:
- Support mental health screenings for individuals navigating same-day access.
- Assist individuals in obtaining needed services in the domains of mental health, social, educational, health, recreational, and related areas to improve child’s functioning.
- Proactively assist individuals to identify barriers regarding access to recommended services.
- Communicate with the members of the individual’s care team to support continuity of care.
- Conduct same day access appointments during scheduled hours.
- Provide consultation and communication with internal and external providers regarding shared clients.
- Develop, in collaboration with the Care Coordination Supervisor and Director of Care Coordination, new processes for supporting families in accessing services and resources as appropriate.

Requirements:
- Must be qualified in at least one of the following ways:
- High School diploma or equivalent required
- Bachelor’s degree or higher in one of the behavioral sciences or related fields from an accredited college or university preferred.
- Certification as a Community Health Worker or Certification as a Family Peer Specialist preferred.
