JOB SUMMARY
Professional member of patient's treatment team who evaluates, assesses, and delivers services according to a written plan of treatment established by a physician. Provides supervision for all services rendered by Physical Therapy Assistants.Evaluations are performed by peers
QUALIFICATIONS
Minimum: Bachelor's degree from an accredited PT program approved by the council on Medical Education of the American Medical Association.
Licensed/registered in the State of Florida as a Physical Therapist.
Minimum of two years Home Health experience as a physical therapist and one year upervisory
Job Responsibilities
Duties of the Registered Physical Therapist (RPT) include, but may not be limited to:
Evaluate patient's level of physical functioning and need for physical therapy services.
- Discuss evaluation with physician to help establish a plan of treatment to assist patient to meetmaximum rehabilitation potential.
- Carry out prescribed treatmentsand/or supervise PTA in carrying out PT
- Establish a home program to be followed by the patient/significant
- Instruct patient/significant other in home
- Observe and report to physician pertinent changein the patient's level of Communicate changeto other team members involved in the patient's care.
- Maintain appropriate documentation of all services as per Agency
- Conduct self in professional manner at all times and in all
- Provide Agency with all required evidence, including copy of license/registration, to allow Agency to verify experience.
- Accept only thoe assignments for which he/he is qualified.
- Comply with all State, Federal, Local and Agency rules, regulations, policies, and procedures.
