ESSENTIAL FUNCTIONS: 

  • Coordinates with other multidisciplinary team members to enhance and achieve positive outcomes with the patient’s diagnosis.
  • Reliable and punctual attendance is essential; expected to be at job as scheduled each scheduled day.
  • Communicate necessary information to others as appropriate.
  • A patient navigation process, driven by needs is established and monitored along their diagnoses trajectory in collaboration with all team members.
  • Monitors and documents quality of care to ensure patient care plan goals are appropriate and achievable. Routinely assesses patient needs and makes appropriate referrals to other clinicians and providers.
  • Facilitates cost effective outcomes by collaborating with physicians, pharmacy or outside providers for medically necessary care.
  • Assesses and discusses funding and insurance issues with client, family, and healthcare providers to enhance cost effective utilization of services and quality outcomes.
  • Provides education and information regarding funding, copay assistance, and insurance concerns as needed.
  • Identifies actual and potential delays in service requests and treatment and communicates back to provider, pharmacy, and other involved staff as indicated.
  • Provides clinical support to prior authorization staff for medical necessity as needed.
  • May set up patients follow up appointments as needed.
  • Responsible for post discharge follow up within Transitional Care Management guidelines
    • Telephone communication within 72 hours post discharge; including medication reconciliation and scheduling patient with provider based on visit acuity (7 days for high risk, 14 days for moderate risk).
  • Required to check daily Admissions, Discharge and transfers (ADTs) through QHN for patients who have visited the Emergency Room or Hospital.
  • Responsible for primary care patient follow-up after patient visit to Emergency Room.
  • Other job duties as assigned.

EDUCATION: 

1.

Education

Associate’s degree in medical field from an accredited school

Licenses

Must obtain Colorado RN/LPN License or Certified/Registered Medical Assistant Certification (unrestricted/restricted) within 6 months of hire

Certifications

Current BLS Certification

4.

Experience

Must have a minimum of two (2) to five (5) years’ experience as a Medical Assistant/RN or related education in hands-on RN duties.  Must have one (1) year of supervisor/lead experience.

  • Must have excellent analytical and problem-solving skills.
  • Must have the ability to apply critical thinking skills and suggest and/or resolve daily staffing needs.
  • Must have excellent leadership ability and people skills.
  • Must have the ability to demonstrate objectiveness and concern for all patients, residents and staff.
  • Must have the ability to communicate effectively and professionally, both verbally and in writing.
  • Requires the ability to deal with a variety of abstract and concrete variables.
  • Requires ability to interpret a variety of instructions furnished in written, verbal, diagram or schedule form.
  • Requires the ability to define problems, collect data, establish facts, and draw valid conclusions.

WAGE RANGE: 

The pay scale for this position is $17.40-$23.38
