Eliot’s Behavioral Health Division provides an integrated and comprehensive network of services to adults with psychiatric disabilities that are evidence based, provide rehabilitation and hope to those in services. Eliot's Behavioral Health Community Partner (BHCP) program is an innovative Care Management approach for MassHealth members with complex medical, behavioral health and social needs who are part of the new Accountable Care Organization (ACO) redesign within Massachusetts.                                                                                         

Responsibilities:

  • Responsible for supervision of BH CP Care Manager staff focused on best practices and strategies to meet identified paperwork targets, Quality Metrics goals, ACO/MCO initiatives, and Key Performance Indicators (KPI). 
  • Responsible for training all new Care Manager hires. 
  • Responsible for participating in Quality Metric and KPI workflow development session(s) with BH CP staff to ensure effectiveness of treatment delivery and program goals are met.
  • Responsible for providing assigned BH CP staff with orientation, training and coaching of BH CP policies and procedures.
  • Responsible for being BH CP liaison to community resources including assisting and supporting BH CP staff with the connection pathways for communicating with internal agency and external collateral contacts.         
  • Assist and support BH CP staff with connecting Members to Social Determinants of Health (SDoH) resources as indicated on screening tools, Care Plan and/or as indicated by Member needs.
  • Participate in relevant internal and external risk management meetings to ensure Member safety concerns are identified and appropriate interventions are identified.
  • Responsible for providing training and coaching to BH CP staff to ensure Comprehensive Assessment, Care Plan, Crisis Prevention Plans and Social Determinants of Health (SDoH) screenings are completed on time as specified by contract.

Qualifications:

  • Must have a bachelor’s degree or master’s degree in psychology, social work or related field with five years’ direct experience in Care Management or Case Management. 
  • Must have a strong commitment to mental health recovery, substance abuse disorders, care coordination, and wellness.
  • Responsible for supervision of Care Management staff and must have at least two years supervisory experience.  
  • Must have skills to establish supportive relationships with persons with severe and persistent mental illnesses, individuals with substance abuse disorders, and those with complex medical conditions. 
  • Must have the ability to communicate clearly and effectively with individuals and engage individuals in assessment, person centered planning, and treatment.
  • Must have the ability to provide flexible services, high quality customer service, and develop partnerships internally and externally.

Schedule: Monday - Friday 9am-5pm

Bonus Breakdown:

  • $1500 after 1 month of employment
  • $1500 after 6 months of employment
