Position Summary:  The Assistant Advanced Heart Failure Program VAD Coordinator will function as a central figure in the overall operations of the Advanced Heart Failure Program including CHF, LVAD, and other related procedures.  Expected roles include direct patient contact and coordination of care, as well as, assisting the AHF providers and LVAD Coordinator with weekly meetings, presentations, and any other tasks that they deem necessary.  Specific responsibilities will include scheduling outpatient evaluations and follow-ups, patient education and maintenance of the ELSO registry.  The successful candidate will work with a multidisciplinary team to ensure optimal patient care and to maximize efficiency in managing the heart failure program.

Experience:  2-3 years’ experience in cardiovascular nursing required; 1+ years’ experience in Heart Failure preferred.

Education:  Minimum Registered nurse RN required.

License and Credentials: Current license in NJ as RN, Current BLS required.

Skills:  Maintains a flexible schedule to ensure patient and staff needs are addressed/ documented in a timely manner.  Proficiency with electronic health records and database management required.  Excellent written and verbal communication skills; proficient with Microsoft Office Suite including WORD, EXCEL, and POWERPOINT; well organized, detail oriented, multi-tasking.

Bi-Weekly Hours: 80

Work Schedule: M-F
