Community Options, Inc. is a national non-profit agency providing services to individuals with disabilities in 12 states.
We are actively seeking a Senior Community Integration Counselor in Schuylkill, PA. This role provides individualized services designed to assist participants in managing and overcoming the difficulties confronted by an individual with a disability living in the community.
Starting pay rate is: $16.50 an hour
We are offering a $500 FT Sign-On Bonus opportunity for New Hires! (Must complete 90 days of satisfactory employment to be eligible)
Responsibilities:
- Assist individuals with daily living skills and the development of community involvement, communication skills, problem-olving skills, and social skills
- Provide ongoing support for community integration efforts
- Assist with developing schedule for each participant to maximize community integration
- Provide trainingto individuals towards achieving program goals
- Complete training to facilitate staff development
- Administer medications and ensures proper completion of necessary documentation
- Plan, develop, facilitate, and participate in individual and group activities with participants, including leisure and volunteer opportunities
- Develop individual day program support plans
- Provide reports on individual progress in relation to overall goals
- Maintains inventory and ensures adequate supplies for program
- Oversees consumer funds as needed
- Monitor the health and medical needs of individuals and immediately report any concerns
- Drive individuals to medical appointments and activities in the community
- Ensure program documentation and billable records are completed accurately and timely
- Build a support network for individuals by developing and maintaining relationships with community members, businesses, and families
- Employees must cooperate with licensee and department staff in any inspection, inquiry or investigation
- Additional tasks and responsibilities may be assigned
Minimum Requirements:
- High School Diploma or GED
- Bachelor’s Degree preferred
- Valid driver’s license with a satisfactory driving record
- Minimum 2 years of relevant experience supporting individuals with intellectual and developmental disabilities
- Ability to establish a comfortable and supportive relationship with individuals receiving supports
- Ability to exercise good judgment and remain calm in crisis situations
- Excellent verbal and written communication skills
- Excellent time management skills
Working Conditions:
- Transport individuals utilizing your own vehicle or company provided vehicles
- Schedule may change due to business needs and may include evening and weekend hours
- May be required to be on-call in cooperation with other management staff
- Frequent lifting, stretching, and other physical exertion may be required
- May be required to lift or move 25+ pounds
- May be exposed to various medical conditions and communicable diseases
