Community Options, Inc. is a national non-profit agency providing services to individuals with disabilities in 12 states. 

We are actively seeking a Senior Community Integration Counselor in Schuylkill, PA. This role provides individualized services designed to assist participants in managing and overcoming the difficulties confronted by an individual with a disability living in the community.

Starting pay rate is: $16.50 an hour

We are offering a $500 FT Sign-On Bonus opportunity for New Hires!  (Must complete 90 days of satisfactory employment to be eligible)

Responsibilities:

  • Assist individuals with daily living skills and the development of community involvement, communication skills, problem-olving skills, and social skills
  • Provide ongoing support for community integration efforts
  • Assist with developing schedule for each participant to maximize community integration
  • Provide trainingto individuals towards achieving program goals
  • Complete training to facilitate staff development
  • Administer medications and ensures proper completion of necessary documentation
  • Plan, develop, facilitate, and participate in individual and group activities with participants, including leisure and volunteer opportunities
  • Develop individual day program support plans
  • Provide reports on individual progress in relation to overall goals
  • Maintains inventory and ensures adequate supplies for program
  • Oversees consumer funds as needed
  • Monitor the health and medical needs of individuals and immediately report any concerns
  • Drive individuals to medical appointments and activities in the community
  • Ensure program documentation and billable records are completed accurately and timely
  • Build a support network for individuals by developing and maintaining relationships with community members, businesses, and families
  • Employees must cooperate with licensee and department staff in any inspection, inquiry or investigation
  • Additional tasks and responsibilities may be assigned

Minimum Requirements:

  • High School Diploma or GED
  • Bachelor’s Degree preferred
  • Valid driver’s license with a satisfactory driving record
  • Minimum 2 years of relevant experience supporting individuals with intellectual and developmental disabilities
  • Ability to establish a comfortable and supportive relationship with individuals receiving supports
  • Ability to exercise good judgment and remain calm in crisis situations
  • Excellent verbal and written communication skills
  • Excellent time management skills

Working Conditions:

  • Transport individuals utilizing your own vehicle or company provided vehicles
  • Schedule may change due to business needs and may include evening and weekend hours
  • May be required to be on-call in cooperation with other management staff
  • Frequent lifting, stretching, and other physical exertion may be required
  • May be required to lift or move 25+ pounds
  • May be exposed to various medical conditions and communicable diseases
