As a Certified Community Behavioral Health Clinic, CNS Healthcare's mission is to serve the people of our communities.
The Specialized Residential Case Manager provides community-based case management services and support to individuals with multiple service needs and high levels of vulnerability in the areas of self-advocacy, education, community awareness, and medication support and management.
Please Note: While this position is primarily community-based, it provides opportunities for some remote flexibility upon the successful completion of the training/onboarding period.
Compensation Range:
The starting pay for this position is $42,330 - $53,950 for limited licensed candidates, and $44,094 - $58,100 annual salary for fully licensed candidates, and is based on non-discriminatory factors such as skills and experience.

- Provide intensive case management services and supports to individuals in a specialized, community residential setting as well as primary and behavioral health integrated services.
- Develop and implement individualized, person-centered plans for individuals to identify needs and interventions to meet those needs that are consistent with guidelines established by funding and accrediting entities.
- Responsible for monitoring, evaluating, and recording individual served progress with respect to treatment goals and develop treatment plans based on stage of change for identified problems, including substance use, and needs implementing stage wise interventions.
- Coordinate housing and treatment with residential providers, including training home staff on treatment plans.
- Establish a collaborative relationship with individuals to best respond to their needs, desires, and interests utilizing formal and informal assessment practices.
- Enhance the ability of the individuals to lead a self-determined life by providing the support and information necessary to build self-esteem and assertiveness, to make decisions, and improve their quality of life.
- Assist individuals in identifying and gaining access to formal and informal supports available in their community and match specific supports and interventions to the unique needs of individual while recognizing the importance of friends, family, and community relationship.
- Identify and use effective advocacy strategies to address and overcome diverse challenges facing individuals served (e.g. human rights, legal, administrative, and financial)
- Assist in the coordination of medication for Individuals as required, including providing education about and monitoring for adverse reactions to medications.
- Coordinate and collaborate with primary and specialty care physicians, community health agencies, and other health care providers.
- Responsible for managing assigned caseload including ensuring that all individual documentation and authorizations are up-to-date, and following admission, transition, and discharge procedures.
- Participate in on-call coverage to ensure access to services during non-standard hours of operation.
- Utilize motivational interviewing strategies and a trauma-informed approach to engage with and provide service to individuals.
- Complete all required documentation completely, accurately, and in accordance with CNS policy.
- Meet direct service productivity standards per agency and program expectations.
- May provide transportation to individuals served as needed.
- Fulfills mandatory education requirements of CNS including evidence-based programs, internal training, and community education.
- Maintain compliance with quality care initiatives including Meaningful Use (MU), Medicare Merit Based Incentive Payment System (MIPS), Commission on Accreditation of Rehabilitation Facilities (CARF), and Certified Community Behavioral Health Clinic (CCBHC) requirements.
- Ensure that the CNS Healthcare standards of service are applied to interactions with individuals served, guests, and staff.
- Assume responsibility for professional growth by current reading and attendance at educational programs and conferences.
- Maintain knowledge and compliance with established policies and procedures, corporate compliance program, code of ethics, applicable federal, state, and local laws and regulations, HIPAA standards, and other regulatory programs.

Qualifications:
Required
- Qualified Mental Health Professional:
- At least a Bachelor’s Degree in Social Work, Sociology, Counseling, Psychology, Criminal Justice, or other Human Services field
- Specialized training (including fieldwork and/or internships associated with the academic curriculum where the student works directly with persons receiving mental health services as part of that experience) OR one year of experience in treating and working with a person who has mental illness.
- Working knowledge and understanding of mental illness, medications used in treating psychiatric disorders, and knowledge and experience with other interventions used to treat psychiatric disorders.
- Ability to write clear, concise reports and the ability to formulate and implement an effective treatment plan.
- Ability to share and teach about the symptoms of substance use disorders that are similar to those of other medical and/ or psychological disorders and how these disorders interact.
- Working knowledge of conflict resolution and crisis intervention.
- Proficient with computers, Microsoft office products (including Outlook, Word, and Excel), and electronic health records (EHRs).

- Ability to work closely with persons with mental or physical limitations.

- Valid and unrestricted Driver’s License
- Transportation or Automobile accessibility with current automobile/vehicle insurance meeting agency policy requirements
Preferred
- Greater than one year of experience in treating and working with adults with severe mental illness (SMI)
- State licensure: Limited-Licensed Bachelor’s Social Worker (LLBSW), Licensed Bachelor’s Social Worker (LBSW), Limited-Licensed Master’s Social Worker (LLMSW), Licensed Master’s Social Worker (LMSW), Limited-Licensed Professional Counselor (LLPC), Licensed Professional Counselor (LPC), Limited-Licensed Marriage and Family Therapist (LLMFT), Licensed Marriage and Family Therapist (LMFT), Temporary Limited-License Psychologist (TLLP), Limited-Licensed Psychologist (LLP), OR Licensed Psychologist (LP)
- Michigan Certification Board for Addiction Professionals (MCBAP) certification as a Certified Alcohol & Drug Counselor (CADC) or Certified Advanced Alcohol and Drug Counselor (CAADC)
- Prior experience in a community mental health setting, specialized residential setting, or in-patient setting.
