JOB TITLE: Licensed Practical Nurse
EMPLOYMENT TYPE: Per diem, Varies
SCHEDULED HOURS: 5:30AM-1130AM; weekends, holidays or as needed
PROGRAM/LOCATION: Pathways OTP, Putnam, CT
OBJECTIVE OF POSITION:
DUTIES & RESPONSIBILITIES:
- Perform initial nursing assessments and document ongoing assessments of patients’ response to treatment.
- Develop, implement and evaluate nursing care plans according to age specific needs of patients. Record progress in a timely manner.
- Responsible for accurate input of computer data.
- Execute the medical and medication regimen of care as prescribed by the physician.
- Provides clients with daily methadone doses and prescribed take-home doses.
- Monitor urine toxicology screens and breathalyzers.
- Administer PPD tests.
- Educate the patient/family toward an understanding of healthy living.
- Respond to medical/psychiatric emergency needs of patients, following CHR policies and procedures.
- Adhere to the standards for Medication Assisted Treatment as maintained by DPH, DMHAS, Joint Commision, D.E.A., and the U.S. Federal Government.
- Monitoring of methadone stock and ordering as described in CHR policies and all regulatory bodies.
- Restocking methadone supply upon delivery.
- Communicate with counselors and Medical Doctor regarding patient status and progress.
- Maintains documentation in compliance with agency and program standards.
- Participates in staff meetings, supervision, trainings and other meetings as assigned.
- Additional duties as required.
QUALIFICATIONS:
Education: Degree/Certification in nursing or in a related health care field preferred.
Experience: none required
Licensure/Certification/Registration: Current RN or LPN license in the State of Connecticut required, Certification in CPR, and must have a valid driver’s license.
COMPENSATION*:
$30/hour
*Our per diem and fee for service positions offer a flat hourly rate.
