ABOUT THE PROGRAM: YOUNG ADULT COMMUNITY TREATMENT TEAM

YACTT is a Multidisciplinary team comprised of the Program Director, Community Support Specialists, Employment Specialists, RN, Outreach Therapists and Care Coordinator. The mission of YACTT is to assist young adults in obtaining the skills they need to lead independent and productive lives. The team provides skill teaching in the areas of life skills, coping skills, symptom management, interpersonal relationships, vocational and educational goals and health and wellness. The team utilizes assertive engagement interventions in order to address barriers to treatment and meet the young adult “where they are at”; outreach is the main mode of service delivery. YACTT emphasizes team work and meets a minimum of three times a week as a full team to coordinate care and to support one another; staff also receive individual supervision with the Program Director who is a Licensed Mental Health professional with an expertise in working with the young adult population. Many of the YACTT clients have experienced significant and/or complex trauma and the team utilizes trauma informed care.

POSITION HIGHLIGHTS:

• Weekly clinical supervision provided; specifically, with young adults.
• Form invaluable and healing relationships with clients.
• Help empower clients to advocate for their own mental health.

DUTIES & RESPONSIBILITIES:

  • Conducts Medical Assessments, LOCUS, Co-occurring Screens and Functional Assessments.
  • Collaborates with individuals to create individualized, person centered treatment plans and documents clients progress during treatment.
  • Provides individual and group education around self-care, medical conditions, mental illness, and medications..
  • Use Motivational Interventions to assist clients in meeting their goals and moving along their paths to recovery, utilizing stage wise interventions
  • Support individual with linking and engaging with natural (family, friends) or community supports
  • Develop crisis plans for clients and work with other staff to respond to crisis situations as needed
  • Provide medication administration for injectable medications and other medication monitoring and coordination needs.
  • Collaborating with Primary Care, Hospitals, Visiting Nurses and Specialty Medical Providers in the community.
  • Provide education, support and coaching around skills of coordinating self-care and medical/behavioral health care needs.
  • Provide skills teaching, such as assisting clients in obtaining/ maintaining housing and utilities; learning how to use transportation services, planning and preparing nutritional meals, engage and communicate with family and friends, be safe in the community, care for children and other family members, and solve everyday problems that arise.
  • Provide intensive skill building interventions and support for self-management, self-advocacy, health and well-being, relapse prevention, and using community resources
  • May perform other assignments and duties, as area needs require.

QUALIFICATIONS: 

Education: Registered Nurse, Bachelors level preferred

Experience: 2+ years of experience with identified population

Licensure/Certification/Registration: Registered Nurse; Valid driver’s license required.

COMPENSATION*:

$49,686 minimum- $69,560 maximum, dependent upon experience. 

*Actual rates are determined at the time of offer and are based off of relevant experience, as well as adherence to our internal equity policy. Many of our positions offer program/location differentials and/or sign-on incentives. (Please note, for part-time positions this range would be pro-rated based on hours worked)
