Duties & Responsibilities:
Compliance and Reporting:
•Adheres to all agency Policies and Procedures, program guidelines, funding, licensing regulations, and accreditation standards.
•Adheres to safety protocols per OSHA and Catholic Charities when conducting job functions.
•Monitors the compliance of safety and health precautions and monitors staff compliance according to all governing bodies’ policies and procedures.
•Provides supervision and guidance to Case Manager HS, Case Aide, and interns in the coordination of intake and case management services to individuals.
•Meets daily with administrative team to discuss opening and closing of beds and units needed to maximize delivery of services.
•Ensures that all files are organized and includes all required documentation.
•Participates in the Performance Quality Improvement (PQI) process and attends monthly PQI Team meetings.
•Ensures appropriate reporting of incident reports in adherence to regulatory agencies and Catholic Charities Policies and Procedures requirements.


Screening & Intake:
•Provides information to those in the community requesting information regarding admission process and coordinate appointments as necessary.
•Conducts pre-screen for potential admissions within 24 hours of referral giving priority to consumers that meet the priority criteria and with urgent needs or emergency situations.
•Provides referrals to individuals who cannot be served or served promptly by connecting them to appropriate resources for the provision of interim services.
•Coordinates and facilitates intakes of individuals, including assessment of possible case management needs.
•Provides prescreening details of individuals pending admission at weekly staffing.
•Collaborates with interdisciplinary team to identify and coordinate services for individuals with unmet medical needs identified as necessary prior to admission or immediately after.

Case Management:
•Provides case management services either directly or by referral to include but not limited in the following areas: employment related services, housing, transportation, social skills training, budgeting and money management, public assistance, interpersonal communications, activities of daily living, medical and dental care, legal assistance, childcare, social, cultural, recreational, and religious activities.
•Maintains accurate and current case record documentation in physical file and agency database.
•Facilitates weekly educational therapeutic groups using material from Living in Balance, Practical Skills, and other topics approved by the Program Coordinator and/or Lead Counselor.
•Submits weekly group progress notes for each group session conducted.
•Facilitates Supplemental Security Income and Social Security Disability Insurance Outreach, Access, and Recovery (SOAR) services in coordination with the interdisciplinary team to eligible individuals.
•Collaborates daily with interdisciplinary team in the development with consumer treatment plan, the provision of information, referral, and confirmed linkage to care for case management needs included in the aftercare and/or treatment plan.
•Provides weekly updates regarding consumer progress.
•Coordinates supervision for individuals when attending court or other necessary appointments.
•Ensures consumers are prepared for independent living upon discharge.

Additional Duties:
•Participates in all required or requested meetings and trainings.
•Completes other tasks as requested by supervisor.


Physical Demands:
•Work is usually sedentary, but may require long periods of standing, walking and/or bending and some light lifting.
•Ability to travel to meeting/training locations.


Educational & Experiential Requirements:
•Bachelor’s degree in social work or related area of study.
•1 year of supervisory experience.
•1 year experience with substance abuse and/or mental health preferred.
