Full Time - Northern Kentucky, Dayton and Cincinnati Area. Seeking experienced nurse practitioner who will evaluate patients before on admission, provide medical oversight, educate on medication management, disease management, and plan of care, and care coordination activities, until the patient is seen by their primary care provider. The nurse practitioner will work regularly with the physicians and nurses in our facility, and will be part of the care team there. We are seeking individuals with nursing home experience or equivalent training, taking care of very ill patients along with the passion and energy to make it successful.

Responsibilities of the Nurse Practitioner:

  • Interact clinically with Physicians, other NP/PAs, case managers and others involved in the acute care episode to understand patient needs in transitional care.
  • Evaluate potential candidates for transitional care management and decide which patients best fit the model of care.
  • In dedicated office space, conduct medication reconciliation and educate patients and caregivers on medication, plan of care and disease management.
  • Work with case managers, PCPs and others to coordinate the transition of care and care services.
  • Maintain a focus on process improvement to optimize the care model.
  •  Must be independent and comfortable being a leader and help the facility in root cause analysis to prevent unnecessary hospitalizations along with improving clinical outcome.
  • Schedule: M-F; approximately a 9-hour day w/lunch; Hours are flexible, but will likely be similar to 8:00am –5:00pm. Hours may change somewhat from day to day depending on needs.
  • On call is not part of the program, but may be instituted if needed.

SPECIAL KNOWLEDGE, SKILLS, ABILITIES, AND/OR TRAINING

  • Ability to work independently and collaboratively
  • Proficiency in teach back methods desirable
  • Excellent organizational skills with the ability to multi-task and prioritize workload in a fast-paced environment
  • Excellent written and oral communication skills
  • Ability to effectively interact and communicate with diverse individuals (e.g., patients, physicians, hospital staff, and other colleagues)

Job Requirements

  • Nurse Practitioner License for Kentucky and/or Ohio
